Mission continued... Needs of homeless persons, offers services to empower homeless persons to deal with the problems they are facing, and works directly with homeless persons to enable them to find suitable housing.

Form 990, Page 2, Part III, Line 4C

The homeless services program the hsp provides the coordinated assessment for anyone in a housing crisis in tippecanoe county. Lthc does an intake, assessment, and triages people to the housing solution which best fits their needs. We provide case managers which create housing stability plans for each client. We offer a variety of supportive services on-site, including: phone/mail services, shower and laundry facilities, day sleep room, meal (breakfast, lunch, and dinner), and transportation assistance. This program is open 7 days/week. Assessment: 1,629 total people; 1,275 adults and 354 children, which includes 173 families; provided 57,580 meals. The homeless services program the hsp provides the coordinated assessment for anyone in a housing crisis in tippecanoe county. Lthc does an intake, assessment, and triages people to the housing solution which best fits their needs. We provide case managers which create housing stability plans for each client. We offer a variety of supportive services on-site, including: phone/mail services, shower and laundry facilities, day sleep room, meals (breakfast, lunch, and dinner), and transportation assistance. This program is open 7 days/week. Provided assessments to 1,459 total people: 1,166 adults and 293 children which includes 129 families; provided 51,536 meals.

Rapid re-housing program the rrh program offers financial assistance for housing and case management for people who are experiencing homelessness. The goal of the program is to house families and individuals that are literally homeless. The case management staff works with each of the clients on their housing stability plans to remain housed. This year, we had other financial resources for those who were impacted by covid. Provided housing and services to 40 families and 40 individuals, including: 100 adults and 92 children. Veteran services program the veteran services program provides help to homeless or at-risk veteran's and their families. The goal is to provide case management services to focus on obtaining permanent housing solutions. Provided housing and services to 89 veterans: 10 families and 85 individuals - 113 adults and 18 children. Total people served in all programs combined in fy '21 1,735 total people in all programs: 1,344 adults, 383 children, 188 families rapid re-housing: 40 individuals and 40 families; 100 adults and 92 children veterans: 85 individuals and 10 families; 113 adults and 18 children; 89 veterans psh all 4 programs: 123 individuals and 16 families; 145 adults and 33 children shelter (crisis, respite, overflow, and interim): 412 people; 6,477 bed nights of shelter hotel emergency shelter: 22 unduplicated familities; 56 adults and 53 children; 457 nights of emergency housing in hotel; 947 bed nights of shelter
