Rapid re-housing program: the rr program offers financial assistance for housing and case management to the homeless with minimal barriers. The goal of the program is to house families and individuals that are in an emergency shelter. The case management staff works with each of the clients on their housing stability plan to remain housed. Provided housing and services to 63 men, 80 women, and 114 children. 52 families and 63 single individuals completed the program. 88% left with income and 97% remained in their housing. 92% of the households came from emergency shelters or places not meant for human habitation and 43% had a physical and/or mental health condition. The average age was 23.6 years old. Average length of stay was 159 days. Supportive services for veteran families: the ssvf program provides help to homeless or at-risk veteran's and their families. The goal is to provide case management services to focus on obtaining permanent housing solutions. Provided housing and services to 126 veterans ~ 161 adults and 34 children. 62% of the adults had a physical and/or mental health challenge. 114 adults completed the program ~ 75% had income and 90% had permanent or supportive housing. The average age was 38 years old. The average length of stay was 114 days.

Accounting standards requires entities to disclose in their financial statements the nature of any uncertainties in their tax position. Tax years including 2012 and later are subject to examination by tax authorities. Areas that irs and state tax authorities consider when examining tax returns of a charity include, but may not be limited to, tax-exempt status and the existence and amount of unrelated business income. The organization does not believe that it has any uncertain tax positions with respect to these or other matters, and therefore has not recorded any unrecognized tax benefits or liabilities. The organization is not aware of any circumstances or events that make it reasonably possible that tax benefits may increase or decrease within 12 months of the date of these financials statements.

LTHC DOES AN INTAKE, ASSESSMENT, AND TRIAGES PEOPLE TO THE HOUSING SOLUTION WHICH BEST FITS THEIR NEEDS. WE PROVIDE CASE MANAGERS WHICH CREATE HOUSING STABILITY PLANS FOR EACH CLIENT. WE OFFER A VARIETY OF SUPPORTIVE SERVICES ON-SITE, INCLUDING: PHONE/MAIL SERVICES, SHOWER AND LAUNDRY FACILITIES, DAY SLEEP ROOM, MEALS (BREAKFAST, LUNCH AND DINNER), AND TRANSPORTATION ASSISTANCE. THIS PROGRAM IS OPEN 7 DAYS/WEEK. FOOD PANTY ~ THE FOOD PANTRY CLOSED ON FEBRUARY 1, 2016. PROVIDED HOUSING AND SERVICES TO 63 MEN, 80 WOMEN, AND 114 CHILDREN. 52 FAMILIES AND 63 SINGLE INDIVIDUALS COMPLETED THE PROGRAM. 88% LEFT WITH INCOME AND 97% REMAINED IN THEIR HOUSING. 92% OF THE HOUSEHOLDS CAME FROM EMERGENCY SHELTERS OR PLACES NOT MEANT FOR HUMAN HABITATION AND 43% HAD A PHYSICAL AND/OR MENTAL HEALTH CONDITION. THE AVERAGE AGE WAS 23.6 YEARS OLD. AVERAGE LENGTH OF STAY WAS 159 DAYS. SUPPORTIVE SERVICES FOR VETERAN FAMILIES: THE SSVF PROGRAM PROVIDES HELP TO HOMELESS OR AT-RISK VETERAN'S AND THEIR FAMILIES. THE GOAL IS TO PROVIDE CASE MANAGEMENT SERVICES TO FOCUS ON OBTAINING PERMANENT HOUSING SOLUTIONS. PROVIDED HOUSING AND SERVICES TO 126 VETERANS ~ 161 ADULTS AND 34 CHILDREN. 62% OF THE ADULTS HAD A PHYSICAL AND/OR MENTAL HEALTH CHALLENGE. 114 ADULTS COMPLETED THE PROGRAM ~ 75% HAD INCOME AND 90% HAD PERMANENT OR SUPPORTIVE HOUSING. THE AVERAGE AGE WAS 38 YEARS OLD. THE AVERAGE LENGTH OF STAY WAS 114 DAYS.
