Mission continued... Needs of homeless persons, offers services to empower homeless persons to deal with the problems they are facing, and works directly with homeless persons to enable them to find suitable housing.

Form 990, Page 2, Part III, Line 4C

The homeless services program: the hsp provides the coordinated assessment for anyone in a housing crisis in tippecanoe county. Lthc does an intake, assessment, and triages people to the housing solution which best fits their needs. We provide case managers which create housing stability plans for each client. We offer a variety of supportive services on-site, including: phone/mail services, shower and laundry facilities, day sleep room, meals (breakfast, lunch, and dinner), and transportation assistance. This program is open 7 days/week. Assessment ~ 1,684 total people ~ 1,262 adults and 422 children, which includes 205 families. Provided 39,617 meals employment services program: 281 guests had an intake with the employment specialist. 171 guests gained employment.

Rapid re-housing program: the rr program offers financial assistance for housing and case management for people who are experiencing homelessness. The goal of the program is to house families and individuals that are literally homeless. The case management staff works with each of the clients on their housing stability plan to remain housed. Provided housing and services to 69 families and 34 individuals, including 117 adults and 141 children. 70 households completed the program. 84% left with income and 96% remained in their housing. 82% of the households came from emergency shelters or places not meant for human habitation and 58% had a disabling condition. The average age was 19.8 years old. The average length of stay was 164 days. Supportive services for veteran families: the ssvf program provides help to homeless or at-risk veteran's and their families. The goal is to provide case management services to focus on obtaining permanent housing solutions. Provided housing and services to 83 veterans: 8 families and 75 single individuals ~ 96 adults and 16 children. 67% of the adults had a disabling condition. 85 adults completed the program ~ 75% had income and 89% had permanent or supportive/veteran housing. The average age was 44.7 years old. The average length of stay was 87 days.

Schedule D, Page 3, Part X

Accounting standards requires entities to disclose in their financial statements the nature of any uncertainties in their tax position. Tax years including 2015 and later are subject to examination by tax authorities. Areas that irs and state tax authorities consider when examining tax returns of a charity include, but may not be limited to, tax-exempt status adn teh existence and amount of unrelated business income. The organization does not believe that it has any uncertain tax positions with respect to these or other matters, and therefore has not recorded any unrecognized tax benefits or liabilities. The organization is not aware of any circumstances or events that make it reasonably possible that tax benefits may increase or decrease within 12 months of the date of these consolidated financial statements. Union place apartments gp, llc is subject to federal and state adjusted gross income tax and supplemental income taxes. It has adopted the provisions of accounting for uncertain tax positions under the fasb accounting standards codification. These provisions prescribe a recognition threshold of more-like-than-not and a measurement attribute for all tax positions taken or expected to be taken on a tax return, in order for those tax positions to be recognized in the consolidated financial statements. For the years ended september 30, 2019 and 2018, union place apartments gp, llc activity generated no taxable income or loss. Therefore no uncertain tax positions have recorded nor any unrecognized tax benefits or liabilities. Tax returns for these years remain subject to examination by tax authorities.

LTHC DOES AN INTAKE, ASSESSMENT, AND TRIAGES PEOPLE TO THE HOUSING SOLUTION WHICH BEST FITS THEIR NEEDS. WE PROVIDE CASE MANAGERS WHICH CREATE HOUSING STABILITY PLANS FOR EACH CLIENT. WE OFFER A VARIETY OF SUPPORTIVE SERVICES ON-SITE, INCLUDING: PHONE/MAIL SERVICES, SHOWER AND LAUNDRY FACILITIES, DAY SLEEP ROOM, MEALS (BREAKFAST, LUNCH, AND DINNER), AND TRANSPORTATION ASSISTANCE. THIS PROGRAM IS OPEN 7 DAYS/WEEK. ASSESSMENT ~ 1,684 TOTAL PEOPLE ~ 1,262 ADULTS AND 422 CHILDREN, WHICH INCLUDES 205 FAMILIES. PROVIDED HOUSING AND SERVICES TO 69 FAMILIES AND 34 INDIVIDUALS, INCLUDING 117 ADULTS AND 141 CHILDREN. 70 HOUSEHOLDS COMPLETED THE PROGRAM. 84% LEFT WITH INCOME AND 96% REMAINED IN THEIR HOUSING. 82% OF THE HOUSEHOLDS CAME FROM EMERGENCY SHELTERS OR PLACES NOT MEANT FOR HUMAN HABITATION AND 58% HAD A DISABLING CONDITION. THE AVERAGE AGE WAS 19.8 YEARS OLD. THE AVERAGE LENGTH OF STAY WAS 164 DAYS. SUPPORTIVE SERVICES FOR VETERAN FAMILIES: THE SSVF PROGRAM PROVIDES HELP TO HOMELESS OR AT-RISK VETERAN'S AND THEIR FAMILIES. THE GOAL IS TO PROVIDE CASE MANAGEMENT SERVICES TO FOCUS ON OBTAINING PERMANENT HOUSING SOLUTIONS. PROVIDED HOUSING AND SERVICES TO 83 VETERANS: 8 FAMILIES AND 75 SINGLE INDIVIDUALS ~ 96 ADULTS AND 16 CHILDREN. 67% OF THE ADULTS HAD A DISABLING CONDITION. 85 ADULTS COMPLETED THE PROGRAM ~ 75% HAD INCOME AND 89% HAD PERMANENT OR SUPPORTIVE/VETERAN HOUSING. THE AVERAGE AGE WAS 44.7 YEARS OLD. THE AVERAGE LENGTH OF STAY WAS 87 DAYS.
