The lincoln center permanent housing supportive housing program houses 24 men and women at 814 north 14th street. The program is designed to provide housing to single individuals who were homeless and have unique challenges to remaining in permanent housing. In fy 2015, lthc housed 19 men and 7 women. 100% of the tenants have a physical and/or mental health condition. 4 tenants left the program-50% left with income and 100% left with permanent housing. Average length of stay was 1,232 days and the average age was 50 years old. The esg rr program offers financial assistance for housing and case management to homeless families with children. The goal of the program is to quickly re-house families that are in an emergency shelter. The case management staff works with each of the shelters and the clients in order to ensure that self-sufficiency goals are being attained. In fy 2015, lthc house 33 men, 59 women and 76 children. 27 families and 22 single individuals completed the program-88% left with income and 98% had permanent housing, 92% of the households came from emergency shelters or places not meant for human habitation and 50% had a physical and/or mental health condition. The average age was 22 years old and the average length of stay was 130 days. The supportive services for veteran families (ssvf) program provides help to homeless or at-rish veteran's and their families. The goal is to provide case management services to focus on obtaining permanent housing solutions. Lthc provided services to 147 adults and 44 children. 116 of the adults were veteran's. 59% of the adults had a physical and/or mental health challenge. 114 adults completed the program- 65% had income and 96% had permanent or supportive housing. Average length of stay was 111 days. The average age was 33 years old.

Schedule D, Page 3, Part X

Accounting standards requires entities to disclose in their financial statements the nature of any uncertainties in their tax position. Tax years including 2011 and later are subject to examination by tax authorities. Areas that irs and state tax authorities consider when examining tax returns of a charity include, but may not be limited to, tax-exempt status and the existence and amount of unrelated business income. The organization does not believe that it has any uncertain tax positions with respect to these or other matters, and therefore has not recorded any unrecognized tax benefits or liabilities. The organization is not aware of any circumstances or events that make it reasonably possible that tax benefits may increase or decrease within 12 months of the date of these financials statements.

IN FY 2015, LTHC HOUSED 5 MEN, 23 WOMEN, AND 53 CHILDREN. 15 FAMILIES COMPLETED THE PROGRAM AND 93% LEFT WITH PERMANENT HOUSING AND 85% LEFT WITH INCOME. 60% OF THE FAMILIES WERE VICTIMS OF DOMESTIC VIOLENCE AND 31% HAD A PHYSICAL AND/OR MENTAL HEALTH CONDITION. THE AVERAGE AGE WAS 16 YEARS OLD AND AVERAGE LENGTH OF STAY WAS 400 DAYS.

EACH PERSON WORKS WITH THEIR CASE MANAGER ON DEVELOPING GOALS, WORKING ON EMPLOYMENT AND/OR EDUCATIONAL ISSUES, AND WHATEVER OTHER TYPES OF BARRIERS A PERSON HAS WHICH LED TO THEIR HOMELESSNESS. IN FY 2015, LTHC HOUSED 25 MEN AND 7 WOMEN. 12 INDIVIDUALS COMPLETED THE PROGRAM AND 91% LEFT WITH PERMANENT HOUSING AND 75% HAD INCOME. 90% OF THE TENANTS HAD A PHYSICAL AND/OR MENTAL HEALTH CONDITION. THE AVERAGE AGE WAS 38 YEARS OLD AND THE AVERAGE LENGTH OF STAY WAS 127 DAYS.

LTHC PROVIDES CASE MANAGEMENT TO ACCESS CLIENT NEEDS AND PROVIDE APPROPRIATE REFERRALS FOR HOUSING. WE OFFER A VARIETY OF SUPPORTIVE SERVICES ON-SITE, WHICH INCLUDE: PHONE/MAIL SERVICES, SHOWER AND LAUNDRY FACILITIES, DAY SLEEP ROOM, MEALS (BREAKFAST, LUNCH AND DINNER), FINANCIAL ASSISTANCE, GAS VOUCHERS AND BUS TOKENS, CLIENT CHOICE FOOD PANTRY, AND A CLOTHING ROOM. IN FY 2015, TOTAL UNDUPLICATED CONSUMERS WAS 142,175(THIS WAS AN 13% DECREASE FROM FY 2014) 9,478 ADULTS AND 7,708 CHILDREN. THE FOOD PANTRY HAD 17,186 HITS AND PROVIDED 25,614 MEALS. CASE MANAGEMENT SERVED 673 ADULTS AND 153 CHILDREN (16% INCREASE FROM FY 2014). 90% LEFT WITH PERMANENT HOUSING OR SUPPORTIVE HOUSING AND 68% HAD INCOME. 68% OF THE ADULTS HAD A PHYSICAL AND/OR MENTAL HEALTH CHALLENGE.

IN FY 2015, LTHC HOUSED 19 MEN AND 7 WOMEN. 100% OF THE TENANTS HAVE A PHYSICAL AND/OR MENTAL HEALTH CONDITION. 4 TENANTS LEFT THE PROGRAM-50% LEFT WITH INCOME AND 100% LEFT WITH PERMANENT HOUSING. AVERAGE LENGTH OF STAY WAS 1,232 DAYS AND THE AVERAGE AGE WAS 50 YEARS OLD. THE ESG RR PROGRAM OFFERS FINANCIAL ASSISTANCE FOR HOUSING AND CASE MANAGEMENT TO HOMELESS FAMILIES WITH CHILDREN. THE GOAL OF THE PROGRAM IS TO QUICKLY RE-HOUSE FAMILIES THAT ARE IN AN EMERGENCY SHELTER. THE CASE MANAGEMENT STAFF WORKS WITH EACH OF THE SHELTERS AND THE CLIENTS IN ORDER TO ENSURE THAT SELF-SUFFICIENCY GOALS ARE BEING ATTAINED. IN FY 2015, LTHC HOUSE 33 MEN, 59 WOMEN AND 76 CHILDREN. 27 FAMILIES AND 22 SINGLE INDIVIDUALS COMPLETED THE PROGRAM-88% LEFT WITH INCOME AND 98% HAD PERMANENT HOUSING, 92% OF THE HOUSEHOLDS CAME FROM EMERGENCY SHELTERS OR PLACES NOT MEANT FOR HUMAN HABITATION AND 50% HAD A PHYSICAL AND/OR MENTAL HEALTH CONDITION. THE AVERAGE AGE WAS 22 YEARS OLD AND THE AVERAGE LENGTH OF STAY WAS 130 DAYS. THE SUPPORTIVE SERVICES FOR VETERAN FAMILIES (SSVF) PROGRAM PROVIDES HELP TO HOMELESS OR AT-RISH VETERAN'S AND THEIR FAMILIES. THE GOAL IS TO PROVIDE CASE MANAGEMENT SERVICES TO FOCUS ON OBTAINING PERMANENT HOUSING SOLUTIONS. LTHC PROVIDED SERVICES TO 147 ADULTS AND 44 CHILDREN. 116 OF THE ADULTS WERE VETERAN'S. 59% OF THE ADULTS HAD A PHYSICAL AND/OR MENTAL HEALTH CHALLENGE. 114 ADULTS COMPLETED THE PROGRAM- 65% HAD INCOME AND 96% HAD PERMANENT OR SUPPORTIVE HOUSING. AVERAGE LENGTH OF STAY WAS 111 DAYS. THE AVERAGE AGE WAS 33 YEARS OLD.
