Mission

The mission of chap is to eliminate health and social disparities in our community by finding those at risk, connecting them to care, and measuring the outcomes. We believe all communities can be transformed through the work of community health workers and the creation of community hubs an accountable care coordination delivery system.

The mission of CHAP is to improve health care and social services for economically disadvantaged persons living in Franklin, Richland and Knox Counties in Central Ohio.

The Organization has adopted the provisions of FASB ASC 740-10-25 that requires the disclosure of uncertain tax positions. There have been no interest or penalties recognized in the accompanying Statements of Financial Position or the Statements of Activities relating to uncertain tax positions. Additionally, no tax positions exist for which it is reasonably possible that the total amount of unrecognized tax benefits will significantly increase or decrease during the next 12 months. The Organization evaluates uncertain tax positions, if any, on a continual basis.

IRS990/Desc0DIRECT CLIENT SERVICES Included in direct client services are CHAPs OIMRI and Help Me Grow programs, along with its involvement in Managed Care Plans. The Ohio Infant Mortality Reduction Initiative OIMRI addresses the barriers African-American children experience, and improves their access to, and utilization of, health care and social services. Help Me Grow is a program for Ohios expectant parents, newborns, infants and toddlers that provides health and developmental services so children start school healthy and ready to learn. Managed Care Plans MCP are health care companies that contract with a variety of medical providers and health care organizations to establish relationships between providers and patients. CHAPs role is to work with the MCPs to help develop a care coordination plan and monitor the clients progress to insure and track a positive outcome.

IRS990/ProgSrvcAccomActy2Grp/Desc0HUB DEVELOPMENT PROJECT CHAP has helped to develop the Community HUB Pathways Model. This is an evidence-based community care coordination approach focused on reaching those at greatest risk, comprehensively evaluating their risk factors and accountably reducing them. The work is done by culturally connected community health workers, nurses and social workers. They reach out to neighborhoods and rural house trailers to engage those populations most at risk. Using specific checklists, they identify risk factors. Working as a team with nurses, social workers and in some cases, physicians, they assure that identified risk factors are addressed with specific Pathways that require confirmed evidence based and best practice intervention. Their work and the Pathways that document the outcome focused steps, assure individuals connect to primary care and prevention services, behavioral health, housing, food, clothing, adult education and employment.
