Mission

EXTENDED HOUSING, INC. IS A 501(C)(3) NONPROFIT ORGANIZATION WHOSE MISSION IS TO PREVENT AND END HOMELESSNESS FOR INDIVIDUALS IN LAKE COUNTY WITH A SERIOUS MENTAL ILLNESS. GOVERNED BY A BOARD OF DIRECTORS, EXTENDED HOUSING, INC. IS CERTIFIED BY THE OHIO MENTAL HEALTH AND ADDICTION SERVICES AND RECEIVES FUNDING AND SUPPORT FROM THE LAKE COUNTY ALCOHOL, DRUG ADDICTION, AND MENTAL HEALTH SERVICES (ADAMHS) BOARD. Our vision is a world in which all individuals with serious mental illness can live as independently as possible and have access to a safe, decent, affordable, supportive place to live. We believe that all individuals - regardless of ability, income, or potential for growth - have intrinsic human dignity and are worthy of respect, compassion and full integration into their community. We recognize and support the individual's right to choose their housing. We value sincerity, integrity, diversity, and excellence in all of our endeavors.

Program Service Accomplishments Housing Coordinators/Support Workers provided 3,835 hours of service to 346 households with severe and persistent mental illness as listed below: 1) Housed 96 persons (85 households) in Supportive Housing (Including Permanent Supportive Housing) in Extended Housing owned units, via properties located throughout Lake County (does not include persons housed in two group homes and one transitional group home, but does include McNaughton and McKinley Grove Apartments). - Member of Painesville Crime Free Housing & Northeast Ohio Apartment Association 2) Provided 124 households with rental subsidies/vouchers to obtain or maintain housing throughout Lake County (including Permanent Supportive Housing). - HUD's Shelter + Care provided rental vouchers to 81 homeless and disabled households. Mental Health services provided from Beacon Health and Signature Health. - Lake County ADAMHS Board funded rental assistance to 42 households (part of Rental Subsidy Program) - Housed 14 veteran households through home funding from the lake county board of commissioners. Funds are targeted for veterans (part of rental subsidy program). 3) Provided 44 households with "no interest" loans. This loan program is for one-time rental assistance or security deposits to help persons/families obtain or maintain housing. - 6 Housing Loan was paid off. 4) number of households successfully transferring off of Extended Housing assistance: - 8 households received a Housing Choice Voucher - 5 households were able to gain enough income to no longer need a Housing Subsidy - 3 households transferred into more appropriate housing (nursing home or group home). Homeless Outreach Workers (PATH Program) provided aggressive outreach to 273 homeless persons in Lake County. Of these: 1) 134 homeless persons were linked with mental health services and resources in the community, such as case management with other mental health agencies. - 85 engaged in mental health services - 29 accessed housing and were no longer homeless Extended Housing owns & manages 19 properties throughout Lake County including: - WILLOUGHBY PLACE AND MADISON PLACE - TWO GROUP HOMES OPERATED BY NORTHCOAST BEHAVIORAL HEALTHCARE. - MCKINLEY GROVE - PERMANENT SUPPORTIVE HOUSING WITH SERVICES ON SITE. - THE EXTENDED HOUSING WELLNESS CENTER - BEGINNING JULY OF 2014, EXTENDED HOUSING BEGAN PROVIDING RESIDENTIAL CARE VIA NORTH COAST HOUSE. -Extended Housing was the recipient of the 2014 Northeast Ohio Apartment Association's Silver Key Award for McKinley Grove - September 2014. VOLUNTEER PROJECTS: EXTENDED HOUSING and its PROPERTIES BENEFITED FROM OVER 700 HOURS OF SERVICE,PRIMARILY THROUGH BUSINESSES VOLUNTEERING THROUGH: -Lake County's United Way Day of Caring (Avery Denison, Lakeland Community College, and Lakewood Federal Credit Union) -Mentor High School Football Team -Progressive Insurance employees -First Church Congregational and many others. -Extended Housing also uses an office volunteer; starting in March 2015, this volunteer works with the agency 20 hours per week. -There are also many hours volunteered by the Board of Directors and community members of Extended Housing's Board Committees.

Individuals requesting services at North Cast House often have a long history of psychiatric hospitalizations and demonstrate a history of being unable to live independently. The program also serves those who are homeless and in need of short term, transitional residential services. North Coast House provides 24 hour staff presence to assist residents in developing and implementing daily living skills and in utilizing community resources. North Coast House provides ongoing individual programming within the residential setting based on the resident's needs and desires. The ultimate goal of North Coast House is for residents to take what they have learned in the home and utilize the skills in an independent living environment.
