990EZ, PART III, PRIMARY EXEMPT PURPOSE:

Community Health Hub (CHH), a Pennsylvania non-profit corporation, is a highly active, community-governed public health service act section 330 compliant organization committed to providing comprehensive, integrated primary care services in northeastern Pennsylvania as a non-discriminatory safety net provider. The mission of CHH is to increase patient and family engagement in the stewardship of comprehensive community centric primary health care. CHH's patient-populated Board of Directors is actively seeking to achieve FQHC designation for the organization to enable CHH to continue to serve a fast-growing, intensely complex low-income population with significant unmet healthcare needs. The cross-industry representation of consumers with community and business leaders on the Board of Directors supports the organization's commitment to a collaborative strategic plan that capitalizes on the collective impact of integrated primary care, dental, mental health and addiction services to positively impact health outcomes. CHH embodies the gold standard model for innovation in the care delivery system through needs-responsive primary care workforce renewal.

Part V, Line 34 -changes to Governing Documents

Provisions were made to amend the bylaws which includes adopting policies and processes to ensure the corporation is in compliance relating to public health services section 330, the structure & composition of the board of directors, and number of voting directorships.
