Chinle Planting HopeChinle, AZ501(c)(3)THIS IS A DYNAMIC ORGANIZATION THAT WORKS TO SUPPORT THE INDIGENOUS COMMUNITY IN MANY AREAS IMPORTANT TO OHF: CHILDREN, HEALTH, ELDERS, HUNGER, ANDEDUCATION. THE ORGANIZATION IS LED BY COMMUNITY MEMBERS AND THOSE DEEPLY EMBEDDED IN THE COMMUNITY IN WHICH THEY SERVE - SO MOST HAVE EXPERIENCED THE NEED AND TRAUMA THAT THEN INSPIRED THEM TO WORK FOR CHANGE. AS A SMALL ORGANIZATION, THEY WOULD BE DEEPLY IMPACTED BY THE GRANT.$25,000
City Youth NowSan Francisco, CA501(c)(3)CITY YOUTH NOW IS A WELL-RESPECTED BAY AREA ORGANIZATION THAT SUPPORTS TEENS, ESPECIALLY THOSE IN FOSTER CARE, WHO HAVE BEEN THROUGH THE JUVENILE JUSTICE SYSTEM, OR ARE IN FINANCIAL NEED, IN A REGION THAT HAS BECOME EXTRAORDINARILY EXPENSIVE FOR MOST FAMILIES. THEY FOCUS ON THE POSITIVE - JOBS TRAINING, EDUCATION, AND THE PERSONAL EMPOWERMENTPROGRAM. THEY ARE ABLE TO DEMONSTRATE SIGNIFICANT IMPACT WHILE KEEPING THEIR BUDGET UNDER $500,000 OFTEN BY PARTNERING WITH REGIONAL LAW ENFORCEMENT, OTHER NONPROFITS, AND COMMUNITY PARTNERS. UNLIKE MANY SOCIAL SERVICE ORGANIZATIONS, MOST OF THEIR REVENUE IS CONTRIBUTED BY INDIVIDUALS AND SMALL FOUNDATIONS.$25,000
Manna Conejo Valley Food BankThousand Oaks, CA501(c)(3)This Organization Has a Strong Anti-hunger Program, Serving Families in Communities Near the Ohf Office. Their Work Not Only Supports Those in Need, Itencourages Other Community Members to Gather and Donate Food Through Food Drives (creating Volunteerism Opportunities). While Their Budget Has Grown, the Need Is Increasing (due to Inflation, Increasing Food Prices, and Food Insecurity) and Fundraising Is Still Tight. So an Unexpected Grant Would Have an Important Impact.$25,000
Solidarity Sandy SpringsSandy Springs, GA501(c)(3)Launched During the Pandemic, This Organization Was Formed by Community Volunteers Who Saw the Extreme Need for Food and Support During the Covidshutdowns and Recovery Period. They Have Very Active Volunteerism Programs, Including for Families With Kids and Groups. the Organization Is Led Byfounder-volunteers With Minimal Staff Support Which Means Most of Their Funding Goes Directly to the Food Pantry Expenses. Also Note That, as of This Writing, Hurricane Helene Is Approaching the Atlanta Region So They May Soon Be Serving Hurricane and Flood Victims. a Grant to This Organization Would Give US the Opportunity in Our Story to Showcase the Role of Volunteerism and Get It Done (by the Founders Who Saw a Need).$25,000

IRS990/Desc0THE OPEN HEARTS FOUNDATION MADE INVESTMENTS THROUGH ITS GRANTMAKING PROGRAM IN ALIGNMENT WITH ITS OPEN HEARTS PHILOSOPHY, WHICH IS INSPIRED BY JANE SEYMOUR'S MOTHER, MIEKE FRANKENBERG. THIS PHILOSOPHY DRIVES THE FOUNDATION'S MISSION FORWARD AND ITS GRANT CYCLE FUNDED $100,000 TO CHARITIES ACROSS THE UNITED STATES. THE FOUNDATION HAS FOCUSED THESE GRANT INVESTMENTS PRIMARILY TO SUPPORT SMALL, EMERGING NONPROFITS WITH BUDGETS OF LESS THAN A MILLION DOLLARS. THROUGH THE FOUNDATION'S EXTENSIVE INTERNAL VETTING PROCESS, 4 CHARITIES RECEIVED GRANTS TO SUPPORT THEIR PROGRAMS AND SERVICES IN ARIZONA, CALIFORNIA, AND GEORGIA. THESE NONPROFITS SERVE HIGH RISK COMMUNITIES IN NEED OF FOOD SECURITY SO THAT CHILDREN AND FAMILIES CAN CONTINUE TO THRIVE. AS PART OF THE GRANTMAKING PROGRAM, THE FOUNDATION SHARES THE MISSION OF EACH FUNDED CHARITY WITH ITS GLOBAL FAMILY OF DONORS AND VOLUNTEERS. ADDITIONALLY, ABOUT HALF OF THE CHARITIES LEVERAGE THE FOUNDATION'S GRANT AS A MATCHING GRANT AND RAISE DOUBLE OR TRIPLE THE FOUNDATION'S GRANT.

IRS990ScheduleI/RecipientTable/PurposeOfGrantTxt0CITY YOUTH NOW IS A WELL-RESPECTED BAY AREA ORGANIZATION THAT SUPPORTS TEENS, ESPECIALLY THOSE IN FOSTER CARE, WHO HAVE BEEN THROUGH THE JUVENILE JUSTICE SYSTEM, OR ARE IN FINANCIAL NEED, IN A REGION THAT HAS BECOME EXTRAORDINARILY EXPENSIVE FOR MOST FAMILIES. UNLIKE MANY SOCIAL SERVICE ORGANIZATIONS, MOST OF THEIR REVENUE IS CONTRIBUTED BY INDIVIDUALS AND SMALL FOUNDATIONS.
IRS990ScheduleI/RecipientTable/PurposeOfGrantTxt1THIS IS A DYNAMIC ORGANIZATION THAT WORKS TO SUPPORT THE INDIGENOUS COMMUNITY IN MANY AREAS IMPORTANT TO OHF: CHILDREN, HEALTH, ELDERS, HUNGER, ANDEDUCATION. THE ORGANIZATION IS LED BY COMMUNITY MEMBERS AND THOSE DEEPLY EMBEDDED IN THE COMMUNITY IN WHICH THEY SERVE - SO MOST HAVE EXPERIENCED THE NEED AND TRAUMA THAT THEN INSPIRED THEM TO WORK FOR CHANGE. AS A SMALL ORGANIZATION, THEY WOULD BE DEEPLY IMPACTED BY THE GRANT.
IRS990ScheduleI/RecipientTable/PurposeOfGrantTxt2THIS ORGANIZATION HAS A STRONG ANTI-HUNGER PROGRAM, SERVING FAMILIES IN COMMUNITIES NEAR THE OHF OFFICE. SO AN UNEXPECTED GRANT WOULD HAVE AN IMPORTANT IMPACT.
IRS990ScheduleI/RecipientTable/PurposeOfGrantTxt3LAUNCHED DURING THE PANDEMIC, THIS ORGANIZATION WAS FORMED BY COMMUNITY VOLUNTEERS WHO SAW THE EXTREME NEED FOR FOOD AND SUPPORT DURING THE COVIDSHUTDOWNS AND RECOVERY PERIOD. A GRANT TO THIS ORGANIZATION WOULD GIVE US THE OPPORTUNITY IN OUR STORY TO SHOWCASE THE ROLE OF VOLUNTEERISM AND GET IT DONE (BY THE FOUNDERS WHO SAW A NEED).

IRS990ScheduleI/SupplementalInformationDetail/ExplanationTxt0IN 2024, THE FOUNDATION AWARDED FOUR GRANTS OF $25,000 EACH, TOTALING $100,000 TO NONPROFIT PARTNERS FOCUSED ON ADDRESSING FOOD INSECURITY. THIS MARKED THE FIRST TIME THE FOUNDATION PRIORITIZED EXISTING PARTNERS FOR GRANTS. GRANT CONSULTANT NICOLE GARZINO WORKED WITH STAFF AND THE COMMITTEE TO RECOMMEND FOUR ORGANIZATIONS TO THE BOARD.
