Schedule I

With sadness, max cure foundation (mcf) must announce the passing of the vice-chairman, co-founder, and chief financial officer, richard l. Plotkin from acute myeloid leukemia in september 2019. We thank those donors, volunteers, friends, and family who expressed an outpouring of support for richard, his vision, and those who survive him. After his passing, the board of directors of mcf elected kevin healy, who served under richard plotkin in managing the daily operations as program coordinator, to the title of acting cfo. Richard is missed every day. Mcf's office operations went fully remote at the end of 2019, having allowed the lease to expire at 560 sylvan ave in englewood, new jersey on the last day of december 2019. Thanks to the charitable support of our board member, dan pfeffer, we have taken up office at 100 wall street in new york, new york and eliminated the need for payment of rent as a foundation expenditure. In 2019, mcf donated a total of $113,019 to other charities for research, advocacy, and to families in mcf's roar beyond barriers financial assistance program. 1. Research: in 2019, mcf granted $28,199 in research donations. A. During the 2019 year mcf has been involved in the production of a white paper and subsequent initiative that calls to attention the correlation between incident rate rise among children and the potential environmental linkages that exist. The childhood cancer prevention initiative is a collaborative nonprofit and business community intent on reshaping the consumer goods in our homes and around our children and addressing the chemicals used in our daily lives that pose a threat to our children's health through carcinogenesis. To that end, $10,500 was granted to the university of massachusetts lowell campus to engage members of their staff in the production of the white paper. An additional $5,000 was provided to clean and healthy new york for their role in producing the white paper. B. Further, $7,500 was provided to the children's cancer therapy development institute through the efforts of the hood to coast relay fundraising effort, spearheaded by volunteer brian jones, to provide funding for acute lymphoblastic leukemia research. C. Finally, mcf granted team mathias $5,199 as part of a fundraising effort by executive director jonathan agin who participated in a penalty kick shootout to fund this pediatric cancer nonprofit honoring their son for osteosarcoma research funding. 2. During the first half of the year, mcf collaborated with the hands on sports foundation to execute a fundraiser that reunited team members of the cleveland crunch soccer team for a 25th anniversary championship reunion game through the collaboration of the otto orf, the chairman of hands on sports foundation. Mcf disbursed half the revenue of the game, after expenses incurred, to hands on sports foundation. A total of $17,874 was granted to hands on sports foundation in support of their mission to provide sports enrichment for children via the game of soccer. 3. The following donations were made in 2019 to other non-profits: temple b'nai abraham mcf sponsored the temple's gala event ($7,500); milana family foundation ($5,000); hope and heroes foundation ($3,600); coalition against childhood cancer mcf is a founding member of this collaborative organization of foundations and others supporting pediatric cancer issues ($300); arms wide open childhood cancer foundation mcf paid to support curefest ($100). Thus, the total paid to other non-profits in 2019 for purposes of spreading awareness of pediatric cancer and mcf was $16,500 4. In 2019 mcf disbursed $50,446 to families through the roar beyond barriers program. A. In all, families have received in excess of $820,000 via the multiple forms of financial assistance that mcf has provided since beginning its initiative to provide funds to families battling cancer in their children. B. In prior years and in 2019, mcf provided gift cards to retailers such as target,
