“Medical respite care - ctc's 50 bed medical respite center provides a time and place to heal for adult men and women experiencing homelessness during periods of acute illness or injury. The ctc respite center admits 400-500 homeless individuals per year for an estimated average of 30-40 days length of stay. While admitted, patients receive medical care from ctc's interdisciplinary team of physicians, nurses, caregivers, and other health providers. Patients are screened for eligibility for health insurance, food assistance, housing, disability, and other income-related entitlements. Each patient collaborates with a dedicated case manager to identify a safe discharge location, with more than 65% of all patients successfully transitioning into living situations other than the street or emergency shelter system. Integrative outpatient treatment - following ctc's initial designation as a federally qualified health center in 2015, the organization has provided integrative, homeless-specialty outpatient services at the parsons family health center and at the downtown family heath center located on the human services campus. Care is provided on both a scheduled and walk-in basis, with same-day services available most days. The scope of services includes comprehensive primary and preventative care, psychiatry, mental health assessment and counseling, substance abuse therapy and social service navigation. Additionally, a 2-exam room mobile unit and intermittent street outreach teams provide care to the community as an extension of the fixed site practices. Program growth - in january 2018, ctc began providing comprehensive primary and preventative care under a health and human services notice of award for supplemental service area funding, involving five new service locations. Subsequent to fiscal year end, ctc opened the fifth new location, a second 50 bed medical respite center situated on the human services campus. Together with all ctc sites of service, ctc hopes to serve more than 10,000 unduplicated patients in calendar year 2019.”
