We remain the best practice model for consumer-lead advocacy. Along with the self-advocates of the Health Committee for People with Disabilities in Allegheny County remaining strong and steadfast and growing, we also have a successful group of self-advocates in Washington County who did a series of legislative visits and held two town halls with electeds. We participated in a Voter Registration effort in three counties: Allegheny, Washington and Westmoreland and hosted a film series and two forums on issues and their impact. Along with advocacy around state budge resources and reduction of stigma, we also were and remain involved in efforts to improve access to oral health care and to reduce out-of-control prescription costs as we involved ourselves in the efforts around Mylan. We held multiple advocacy trainings in multiple counties training over 300 persons. We continue to establish ourselves as the consumer-lead advocacy voice on local, statewide and national issues.”

In accordance with general accepted accounting principles, the Coalition accounts for uncertain tax positions using a recognition threshold of more-likely-than-not to be sustained upon examination by the appropriate taxing authority. PHAN is a statewide coalition of more than 60 organizations representing the faith, legal services, public health, immigrant and disability rights communities. PHAN works to protect high quality health insurance coverage for individuals and businesses and to expand coverage to the uninsured while also providing health coverage enrollment assistance and related services. It is the leading coalition providing education and outreach on health care reform and has provideddirect enrollment assistance to uninsured Pennsylvanians since 2013. PHAN employs navigators across the state who help individuals and families apply for health insurance affordability programs like the Marketplace (healthcare.gov), Medicaid, and CHIP. Our staff is available evenings, weekends, and during normal business hours, in-person and through oir statewide helpline, to help Pennsylvanians navigate the health care landscape. |

| IRS990/ProgSrvcAccomActy2Grp/Desc | 0 | Outreach, Education and Enrollment in Healthcare Coverage:The Consumer Health Coalition (CHC) employs licensed healthcare Navigators who assist underserved and at-risk consumers with accessing public healthcare programs. Navigators assist with in-person Qualified Health Plan counseling assistance through the Federally Facilitated Marketplace and/or by telephone or in-person with applying for Medical Assistance and the CHIP program.CHCs education, outreach, and enrollment efforts extends throughout Southwestern Pennsylvania. Navigators partner and assist in a variety of settings from community venues to jails to assist individuals and their families in obtaining affordable, accessible and quality healthcare coverage. CHCs staff, some of whom are bilingual, also assists families with plan reauthorization or renewal of coverage, locating providers, billing problems, reaching their caseworkers to resolve issues, applying for free and low-cost pharamaceutical programs and navigating patient assistance programs. One of CHCs primary programs is its Helpline, a telephone helpline through which consumers receive assistance in accessing health care and health coverage.In 2016, CHC completed 1,477 applications for 1,978 consumers through its Helpline. Navigators provided healthcare-related education and counseling to 1,547 additional individuals. Further, CHC made 5,140 follow-up calls to consumers to ensure that their applications were completed, to address any issues, and to determine whether they were approved. CHC participated in 132 outreach, education, and enrollment events reaching more than 10,332 individuals. In addition to the navigator services CHC is committed to addressing health inequities, especially in Allegheny County and the city of Pittsburgh. CHC has formed a new coalition of leaders and community stakeholders, including consumers, to address global opportunities between Pittsburgh and Glasgow, Scotland defining our similarities and population disparities in health and ways to address these for the resilience and sustainability of our communities. CHC has formed specific outreach and education activities around chronic diseases and conditions, highlighting disparities and addressing the social drivers of health. |
