Mission

To inspire consumer movement to enhance access to quality, affordable health care in Southwestern Pennsylvania.

Major Activities

Activity 2
Health committee for people with disabilities the health committee for people with disabilities (hcpd) continues to be a leader in the training and leadership development of persons with disabilities and organizations serving them within the counties of southwestern pennsylvania. The full group and each subcommittee met monthly. This past year 625 individuals were trained and educated in the counties of allegheny, beaver, washington and westmoreland. We continued our training and education on being a proactive patient. These trainings were presented to 474 persons around allegheny county. In addition, we completed a photo voice project in westmoreland county titled; what i need to live well. It drew a crowd of over 140 persons and the media. Artists in allegheny and washington counties had a month-long exhibition of their artwork at the main branch of the carnegie library.
Activity 3
Expanding health care access as the affordable care act (aca) moves toward full implementation, we held an aca house party on the anniversary of the acts passage. We held two conference calls around the aca and the supreme courts debate and ultimate upholding of the law. A presentation on the aca was given to a group of consumers in beaver county and a discussion was held with members of the pittsburgh health corps. As the focus turned toward medicaid expansion we brought together local advocates and held two informational and planning meetings. These lead to a community meeting on medicaid expansion that was attended by over 50 persons. A consumer voice in the health care quality movement we concluded our successful pa campaign for better care (pa-cbc) in june. Prior to wrapping up the campaign, we lead four lift up your voice advocacy trainings for vulnerable seniors living with chronic conditions and caregivers. We trained 48 persons and conducted trainings in beaver, pittsburgh, and greensburg. We held statewide, monthly conference calls for members of the pa-cbc that featured a topic expert dealing with issues ranging from the coleman model for successful transitions, medicare and medicaid, home and community-based services and the affordable care act. The healthy hospitals workgroup met monthly and sharpened its focus on reduction of hospital readmissions. We created a poster that highlighted the work of the group and was part of an event sponsored by robert morris university. We held a series of six focus groups (three with seniors and three with caregivers) in pittsburgh and westmoreland counties dealing with the transition from hospital to home. Based on the data gathered, a final paper was prepared and shared with our partners.

FAMILIES WERE ASSISTED WITH APPLICATIONS FOR THE MEDICAL ASSISTANCE AND CHIP PROGRAMS IN PENNSYLVANIA; DIRECTED TO FREE AND LOW-COST MEDICATION AND MEDICAL EQUIPMENT PROGRAMS, AND OTHER FREE AND LOW-COST SERVICES. WHEN ASKED HOW THE CALLER HEARD ABOUT OUR SERVICES, MOST STATED THAT THEY HEARD ABOUT US THROUGH A PARTNER ORGANIZATION. CHC GOES OUT INTO THE COMMUNITY AND IDENTIFIES ORGANIZATIONS AND VARIOUS PARTNERS THAT SERVE VULNERABLE POPULATIONS, AND CONDUCTS FREE TRAININGS FOR THE ORGANIZATIONS STAFF ON BOTH ELIGIBILITY AND ENROLLMENT IN PUBLIC HEALTH INSURANCE AND OTHER PUBLIC HEALTH PROGRAMS. WE CALL THIS TRAINING OUR 100% PLEDGE CAMPAIGN. THE 100% PLEDGE PROGRAM TRAINS STAFF AT THESE ORGANIZATION AND PARTNER SITES TO BE APPLICATION ASSISTERS, SO THAT THEY CAN COMPLETE APPLICATIONS FOR THEIR CLIENTS. UPON COMPLETION OF THE TRAINING, THE ORGANIZATION IS ASKED TO SIGN A PLEDGE WITH CHC. WE CALL THIS TRAINING OUR 100% PLEDGE CAMPAIGN. THE 100% PLEDGE PROGRAM TRAINS STAFF AT THESE ORGANIZATION AND PARTNER SITES TO BE APPLICATION ASSISTERS, SO THAT THEY CAN COMPLETE APPLICATIONS FOR THEIR CLIENTS. UPON COMPLETION OF THE TRAINING, THE ORGANIZATION IS ASKED TO SIGN A PLEDGE WITH CHC. THE PLEDGE SAYS THAT THE ORGANIZATION WILL ASK EVERYONE THEY COME IN CONTACT WITH INCLUDING STAFF, VOLUNTEERS AND CLIENTS, IF THEY HAVE HEALTH INSURANCE, AND IF THEY COME ACROSS AN INDIVIDUAL THAT DOES NOT, THEY COMMIT TO ASSIST THEM USING THE TOOLS PROVIDED BY CHC OR REFER THEM TO CHCS FREE HELPLINE FOR ASSISTANCE. OVER 1300 FLYERS, BROCHURES, AND INFORMATIONAL MATERIALS WERE DISTRIBUTED THROUGH OUTREACH EVENTS INCLUDING: HEALTH FAIRS, COMMUNITY DAYS, FOOD BANKS, AND THROUGH ON-SITE OUTREACH AT BUSINESSES IN ALLEGHENY AND SURROUNDING COUNTIES. ADDITIONALLY, HELPLINE STAFF TRAINED 292 INDIVIDUALS ON CHCS HELPLINE SERVICES, THE PATIENT PROTECTION AND AFFORDABLE CARE ACT, AND OTHER PERTINENT HEALTH POLICY RELATED ISSUES.

WE TRAINED 48 PERSONS AND CONDUCTED TRAININGS IN BEAVER, PITTSBURGH, AND GREENSBURG. WE HELD STATEWIDE, MONTHLY CONFERENCE CALLS FOR MEMBERS OF THE PA-CBC THAT FEATURED A TOPIC EXPERT DEALING WITH ISSUES RANGING FROM THE COLEMAN MODEL FOR SUCCESSFUL TRANSITIONS, MEDICARE AND MEDICAID, HOME AND COMMUNITY-BASED SERVICES AND THE AFFORDABLE CARE ACT. THE HEALTHY HOSPITALS WORKGROUP MET MONTHLY AND SHARPENED ITS FOCUS ON REDUCTION OF HOSPITAL READMISSIONS. WE CREATED A POSTER THAT HIGHLIGHTED THE WORK OF THE GROUP AND WAS PART OF AN EVENT SPONSORED BY ROBERT MORRIS UNIVERSITY. WE HELD A SERIES OF SIX FOCUS GROUPS (THREE WITH SENIORS AND THREE WITH CAREGIVERS) IN PITTSBURGH AND WESTMORELAND COUNTIES DEALING WITH THE TRANSITION FROM HOSPITAL TO HOME. BASED ON THE DATA GATHERED, A FINAL PAPER WAS PREPARED AND SHARED WITH OUR PARTNERS.
