Activity 2

Hiv/aids services persad provides both care services to hiv+ individuals and prevention services to individuals at high risk for infection. Persad has been providing hiv/aids services since the early 1980s. The agency provides a variety of care services including counseling for the newly diagnosed as well as long term survivors, mental heath interventions, and substance abuse treatment. Hiv/aids care services also include support groups, coping with grief and loss, and strategies to improve individual's ability to adhere to treatment and medication regimens. Individuals served in hiv/aids services demonstrate improved coping skills, report increased networks of support, and show improved self care management. Persad provided 74 individuals with 1232 hours of care this year. Persad's hiv prevention services target several high risk populations including gay men, men who have sex with men (msm), young adult men of color, street youth, and hiv+ individuals. Persad provides different interventions to these groups that are specifically designed and proven effective for the population. The demonstrated effective interventions that persad uses include: mpowerment (for young men of color), street smart (for street youth), healthy relationships (for hiv+ individuals to prevent re- infection), and respect (for gay men and msm). Individuals who participate in hiv prevention services benefit from knowing their hiv status, increase their awareness and knowledge of hiv disease, and recognize and reduce the behaviors that put them at risk for infection. 2954 participants received 5564 hours of intervention.

Activity 3

Runaway and homeless youth street outreach services: runaway and homeless youth (rhy) street outreach services are a critical support to youth who are on the streets with no stable living situation. Disproportionate numbers of runaway and homeless youth are lgbtq and have been thrown out of their homes or runaway because of conflicts with their families over their gender or sexual orientation (30-35% of a rhy are lgbtq). Persad provides street level intervention to assist youth with tangible aid, and to develop relationships with them that allow them to accept assistance and to find solutions to leave the street for stable housing. Our outreach staff provide referral connections and guide youth to shelters, independent living or facilitate their return to families with counseling and support. During 2011-2012, the program assisted 554 youth with 6234 hours of outreach services and assisted 51 youth to leave the streets to safe permanent housing.
