FORM 990, PART VI, SECTION B, LINE 12C:

The agency has a conflict of interest policy that pertains to all key employees, officers, and directors. Any of these individuals who may be involved in a transaction in which there is a possible conflict of interest (including any related party transaction) is required to report it to the chair of the board's governance committee. If the possible conflict involves the governance committee chair, it should be reported to another member of the committee. The governance committee will determine whether conflict exists and, if so, whether the transaction should be permitted. Financial Statement Notes

PART X, LINE 2:

Community access, inc. (the "reporting organization") has not taken an uncertain tax position that would require provision of a liability under accounting standards codification ("asc") 740, "income taxes". Under asc 740, an organization must recognize the financial statement effects of a tax position taken for tax return purposes when it is more likely than not that the position will not be sustained upon examination. The reporting organization does not believe there are any material uncertain tax positions and, accordingly, it will not recognize the financial statement effects for unrecognized tax positions. The reporting organization has filed for and received income tax exemptions in the jurisdictions where it is required to do so. Additionally, the reporting organization has filed irs form 990 tax returns, as required, and all other applicable returns in jurisdictions when it is required.

OUR TENANTS INCLUDE MANY PEOPLE WITH LIVING WITH MENTAL HEALTH CONCERNS WHO HAVE ALSO EXPERIENCED HOMELESSNESS. MANY OF OUR SITES INCLUDE AFFORDABLE UNITS FOR FAMILIES, A MODEL THAT HAS PROVEN HIGHLY SUCCESSFUL IN PROMOTING COMMUNITY INTEGRATION. IN ADDITION TO PROVIDING HOUSING, WE OFFER A NUMBER OF PROGRAMS TO IMPROVE OUR RESIDENTS QUALITY OF LIFE, INCLUDING PET ACCESS, A PET ADOPTION PROGRAM, AND THE URBAN FARMING INITIATIVE, WHICH OFFERS PARTICIPANTS OPPORTUNITIES TO WORK TOGETHER TO PLANT, NURTURE AND HARVEST FOOD, AS WELL AS INCREASING AWARENESS OF NUTRITION AND FOOD JUSTICE ISSUES EFFECTING OUR COMMUNITY.
