The vision is to develop the capacities of adults and children to sustain healthy lifestyles by providing services designed to foster addiction-free lives, emotional wellness, financial stability and safe communities.

The center for prevention & counseling has its form 990 prepared by an outside accounting firm and has established the following review process to ensure that the information reported is complete and accurate. When the form 990 has been prepared, reviewed by management and is ready to be filed with the internal revenue service, it is provided to the members of the organizations governing body for any comments prior to its submission. The governing body is provided with a reasonable amount of time to review the form 990. Any comments are then grouped, summarized and provided through management to the outside accounting firm. Any applicable issues are addressed until the return is finalized and approved for filing.

Form 990, Part VI, Section B, Line 12C

The center for prevention & counseling currently has in place a conflict of interest policy which it regularly monitors and enforces. The board mandates that all members of management and the governing body annually sign a conflict of interest policy and disclose any potential or actual conflicts that may exist. If a potential or actual conflict of interest exists, governing body and management will investigate the issue. If a conflict of interest is determined to exist management and the governing body will be notified immediately. The member will not be allowed to vote or be a part of any decisions about any such transactions that have to do with the conflict until such time that there is no longer a conflict.

An annual review of the executive director's performance is completed by the board under the leadership of the board president. The board sets and approves the executive director's salary. The executive director sets all other salaries and provides employees with annual reviews. All salaries are included in the organization's annual budget which is approved by the board finance committee.

PART IV, LINE 2B:

The organization holds certain funds on behalf of its clients which have been received from grantors and are subject to grant funding requirements to maintain these funds.

PART X, LINE 2:

The organization qualifies as a tax-exempt organization under internal revenue code section 501(c)(3) and state of new jersey title 15, new jersey nonprofit corporation act. Accordingly, the organization is exempt from federal and state income taxes. These sections enable the organization to accept donations, which qualify as charitable contributions to the donor. The organization is not classified as a private foundation. The organization follows accounting for uncertainty in income taxes, in accordance with gaap. The determination of uncertain tax positions uses tax judgments which are based on the requirements for maintaining tax-exempt status and on the filing of various information returns. The organization files federal information tax returns with the irs and state information returns with the state of new jersey. The organization is subject to income tax examinations at any time within three years from the latest filing date for federal and four years from the latest filling date for new jersey.

Part V, Line 4 Intended Uses of the Endowment Fund

The intended use of the organization's endowment fund is to provide counseling to low-income individuals and families unable to pay for life-saving treatment services.

THE ORGANIZATION IS NOT CLASSIFIED AS A PRIVATE FOUNDATION. THE ORGANIZATION FOLLOWS ACCOUNTING FOR UNCERTAINTY IN INCOME TAXES, IN ACCORDANCE WITH GAAP.
