Form 990, Part VI, Line 11B: Form 990 Review Process

Once the draft of the annual 990 is prepared by a contracted cpa in collaboration with the firefighters burn institute accounting staff and executive director, it is reviewed by the president and secretary/treasurer of the board of trustees. The 990 is also provided to the entire board of trustees for review and comments prior to being filed.

Form 990, Part VI, Line 12C: Explanation of Monitoring and Enforcement of Conflicts

The firefighters burn institute regularly monitors and enforces compliance with the conflict of interest policy. Any member of the board, staff member, committee member, volunteer or donor can request to have the issue of a conflict of interest addressed by notifying the firefighters burn institute executive director or the president of the board of trustees. Board members are made aware of their responsibility to disclose any proposed activities which present or may be a conflict of interest to the president or executive director and have them approved in advance by the board of trustees. If deemed necessary, outside professionals, including attorneys are contracted to review any perceived conflicts. A statement is signed by each board of trustee member annually, agreeing to the terms of the conflict of interest policy. Enforcement is the responsibility of the board of trustees under the guidance of the current president of the board.

Form 990, Part VI, Line 15B: Compensation Review and Approval Process for Officers and Key Employees

When determining compensation for firefighters burn institute executive director, ceo, top management and other officers or key employees, an annual salary proposal is submitted to the president and the secretary treasurer of the board of trustees in written format by the ffbi executive director, along with current salary information for comparable job descriptions in similar geographic areas for similar agencies. The determination of compensation is made after deliberation by the board of trustees as a whole, once compensation comparisons, projected budget, staff performance, job descriptions and other relevant information have been reviewed.

Form 990, Part VI, Line 19: Other Organization Documents Publicly Available

The firefighters burn institute adheres to a public disclosure policy, which includes retaining a binder on the premises at 3101 stockton blvd, sacramento, ca 95820, containing current governing documents, conflict of interest policy and financial statements.
