Form 990 governing body review Part VI line 11

The Form 990 is prepared with the assistance of an independent certified public accountant in coordination with the Operations Manager. Each board member completes a Form 990 Board Member Questionnaire which provides information on conflicts of interest and any relationships. Prior to filing, the Form 990 is reviewed by the President and the Treasurer of the Board and the Executive Vice President for Operations and Artistic Director. A copy of the Form 990 is provided to the Board of Directors before it is filed.

This State program helps many communities considered by Florida Department of Cultural Affairs to be underserved in the arts to be able to bring our Company in at a reduced cost. We also benefit the children of these areas through Community Nutcrackers. The Executive Artistic Director auditions and trains local children for the parts in the Nutcracker. We perform at a number of senior communities and in Ft. Lauderdale, Florida we present eight school shows serving over 3400 children.

IRS990/ProgSrvcAccomActy3Grp/Desc0Ballet Programs: Outreach- There are four outreach programs: DANCIN is a program that provides children ages 8-17 opportunities to perform in venues such as arts festivals, retirement facilities and schools and to create work and set it on their colleagues. DANCE PARTNERS is a program designed to bring at-risk or disadvantaged persons to Mainstage Productions. Dance Alive works with United Way and the Florida Department of Children and Families to bring this program to over 1,500 individuals at no cost. DISCOVERING DANCE, The Mainstage Experience is a program whereby approximately 3,500 children from Alachua and surrounding counties are brought to daytime Professional Mainstage productions at the Phillips Center for the Performing Arts as well as directly to the schools. SENIOR MOMENTS, is designed to provide life-long learning activities for seniors. Dance Alive presents lectures, demonstrations, seminars and performances to three facilities in Gainesville FL.
