Activity 2

Supports coordination provides a full array of required supports coordination services and the delivery of person-centered planning for persons with a wide array of disabilities in varied work and residential situations to ensure the responsible delivery of services and goals along with families and others. The objective of the program is to work with families, care providers, and natural supporters (friends, neighbors, etc.) and respond as appropriate to changes in the beneficiary's life. Assist the beneficiary in self employment, individual job placement, group employment, and our own resource center based employment. Assisting in all matters of entitilements, such as social security, medicaid, ssi, medicare, home help services, section 8 housing, food stamps, wage records, spend-down, medical determinations, applications, and appeals. We assisted 584 beneficiaries under this program. They may have participated in other programs we administer.

Activity 3

Supported employment is a combination of ongoing support services and paid employment that enables the beneficiary to work in the community. The objective of the program is community based, taking place in integrated work settings where workers with disabilities work alongside people who do not have disabilities. We help the individual find a job which matches their interests and experience. We place our beneficiaries in retail, janitorial, food services, recycling, restaurants, hotels, animal care, retirement communities, child care, municipalities, assembly, non profits, and packaging. We supported 283 beneficiaries in their pursuit of employment in the community. We helped develop an employment plan, preparing a resume, job lead development, locating and contracting prespective employers, etc.
