Mission

The organization's mission is to advance patient safety worldwide by empowering the healthcare community, including consumers, to prevent medication errors.the organization accomplishes its mission through its interdisciplinary efforts to:- collect and analyze reports of medication-related hazardous conditions, near-misses, errors, and other adverse drug events- disseminate timely medication safety information, risk-reduction tools, and error-prevention strategies- educate the healthcare community and consumers about safe medication practices- collaborate with other patient safety organizations, educational institutions, governmental agencies and other healthcare stakeholders- advocate the adoption of safe medication standards by accrediting bodies, manufacturers, policy makers, regulatory agencies, and standards-setting organizations- conduct research to provide evidence-based safe medication practices

Part X, Line 2:

In accordance with ASC Topic 740 Accounting for Uncertainty in Income Taxes, the Organization has evaluated its tax positions. A tax position is recognized as a benefit only if it is more likely than not that the tax position would be sustained in a tax examination, with a tax examination being presumed to occur. The amount recognized is the largest amount of tax benefit that has a likelihood of being realized on examination of more than fifty percent. For tax positions not meeting the more likely than not test, no tax benefit is recorded. Under the more likely than not threshold guidelines, the Organization believes no significant uncertain tax positions exist, either individually or in the aggregate, that would give rise to the non-recognition of an existing tax benefit. In addition, the Organization had no material unrecognized tax benefits or accrued interest and penalties.

The biweekly ISMP Medication Safety Alert! The monthly Community/ Ambulatory Care edition and Nurse Advise-ERR newsletters give pharmacists, pharmacy technicians, nurses, physicians, and other health professionals recommendations that they can use in their own practice settings. The monthly ISMP Medication Safety Alert! Long-Term Care Advise-ERR provides timely medication safety information to staff working with resident populations that are often elderly, frail, dependent, complex in nature, and fraught with many different illnesses and dysfunctions. Safe Medicine, published every other month, is the only consumer health newsletter focusing specifically on error prevention. ISMP also publishes educational columns in several healthcare professional journals and publications and comprehensive books on safe medication use for healthcare practitioners.

Onsite hospital and health system risk assessments by a multidisciplinary team of practitioners result in safe practice recommendations for medication error prevention and follow-up implementation and support services. On an annual basis, these onsite visits directly impact facilities representing more than 5,000 inpatient beds and hundreds of thousands of emergency department visits. In addition, ISMP consultants provide similar assessment and support to outpatient pharmacies nationwide, representing tens of thousands of prescriptions filled each year. Unlike other consultants, ISMP constantly updates its evidenced-based prevention strategies with real-time information gained from ongoing analysis of thousands of reports submitted to its national voluntary error reporting programs.

In addition, ISMP hosts medication safety fellowships and onsite mentoring programs for medication safety officers and international healthcare practitioners. IRS990/ProgSrvcAccomActyOtherGrp/Desc0Various Programs:Error Reporting and Analysis:Nearly 40 years ago, the voluntary ISMP National Medication Errors Reporting Program (ISMP MERP) was formed to learn about errors happening across the nation, understand their causes, and share "lessons learned" with the healthcare community. Each year, the program receives hundreds of reports from healthcare practitioners, and that information is used to bring about large-scale changes in medication safety. The Institute also reviews all medication error reports submitted by healthcare facilities to the Commonwealth of Pennsylvania Patient Safety Authority, and established the first consumer error reporting system through its consumer website (www.ConsumerMedSafety.org) as well as a National Vaccine Errors Reporting Program (ISMP VERP). ISMP was named one of the first federally certified Patient Safety Organizations (PSOs), providing the highest level of legal protection and confidentiality for the data and error reports it receives. ISMP's QuarterWatch program analyzes errors and serious side effects reported to the U.S. Food and Drug Administration and issues reports that identify safety problems with specific medicines. Research and Best Practices Development:In addition to disseminating information on safe medication use gained through its reporting programs and other sources, ISMP conducts original research to help provide evidence-based solutions and develops guidance and prevention strategies to prevent errors. As a recent example, ISMP launched the 2014-2015 Targeted Medication Safety Best Practices for Hospitals to identify, inspire, and mobilize national action on recurring safety problems. ISMP has also conducted national medication safety self assessments for acute care hospitals in 2000, 2004, and 2011 and an international assessment for oncology medication safety in 2012. In 2014, the Institute published an article on the state of safe medication practices over the last decade based on findings from its self assessments. The article, published in The Joint Commission Journal on Quality and Patient Safety, also provided a list of national safety priorities that merit greater attention.
