ISMP also reviews all medication error reports submitted by healthcare facilities to the Commonwealth of Pennsylvania Patient Safety Authority, and established the first consumer error reporting system through its consumer website (www.ConsumerMedSafety.org) as well as a National Vaccine Errors Reporting Program (ISMP VERP). ISMP was named one of the first federally certified Patient Safety Organizations (PSOs), providing the highest level of legal protection and confidentiality for the data and error reports it receives. ISMP's QuarterWatch program analyzes errors and serious side effects reported to the U.S. Food and Drug Administration and issues reports that identify safety problems with specific medicines. |
