Mission

To provide quality behavioral healthcare designed to empower and promote wellness for individuals, families, and communities.

The organization provides caring and professional behavorial healthcare to individuals in need of recovery and personal wellness. The organization provides outpatient counseling and medication management to low-income members of the community, including montgomery, chester, berks and other surrounding counties in southeast pennsylvania. The organization serves over 5,000 adults, children and families each year. In addition, they maintain active collaborations with ten area school districts, the judiciary system, several united ways, the tri-county community network and other human service agencies. The organizations mission is to provide quality behavorial healthcare designed to empower and promote wellness for individuals, famililies and communities.

The chief executive director and chief financial officer of creative health services, inc. Along with the finance committee review the form 990 before sending it to the internal revenue service.

Form 990, Part VI, Section B, Line 12C

The chief operating officer of creative health services, inc. Sends a letter to each board of director during the first week of july requesting that the conflict of interest statement and disclosure statement be completed, signed and returned as soon as possible. If the completed forms are not received by the september board meeting, then the cfo follows up with the board of director personally to have the forms completed before the end of the september board meeting.

The personnel committee meets with the ceo and cfo annually. Their performance is reviewed and goals for the upcoming next year are determined. Both the finance and personnel committees make recomendations to the baord of directors based on the above reviews and comparisons made to other comparable agencies.
