Mission

Maine quality counts is transforming health and healthcare in maine by leading, collaborating, and aligning improvement efforts.

Major Activities

Activity 2
Patient centered medical home pilot this pilot offers a promising approach to transforming healthcare in maine to create dramatic and sustainable improvements in care. The maine quality forum, maine quality counts, and the maine health management coalition have convened a multi-stakeholder effort to develop, implem, and evaluate themaine patient centered medical home pilot as the first step in achieving statewide implementation of the medical home model. The pilot provides support for 26 primary care physician practices from across the state to achieve practice transformation, and promotes the payment reform needed to achieve this system transformation. The ultimate goal of this effort is to sustain and revitalize primary care both to improve health outcomes for all maine people, and to reduce overall healthcare costs.
Activity 3
Other various programs to coordinate disparate efforts to improve health, promote consistent delivery of high quality care, improve access to care and contain healthcare costs. Programs include community care teams, consumer engagement, behavioral health integration, quality counts for kids, and transforming care at the bedside.

THE ROBERT WOOD JOHNSON FOUNDATION SEEKS TO HELP GROUPS OF STAKEHOLDERS IN TARGETED COMMUNITIES ACROSS THE COUNTRY SET AND ACHIEVE AMBITIOUS GOALS TO IMPROVE THE QUALITY AND VALUE OF HEALTHCARE. RQS WILL SUPPORT INITIATIVES LED BY LEADERSHIP TEAMS TO WORK TOWARDS DRAMATIC, SUSTAINABLE IMPROVEMENTS IN THE QUALITY AND VALUE OF THEIR COMMUNITIES HEALTHCARE. NUMEROUS PROJECTS ARE EXECUTED AS PART OF THIS INITIATIVE INCLUDING A HOSPITAL QUALITY NETWORK, A PATIENT FAMILY LEADERSHIP TEAM, AND OTHERS.
