Mission and Program Overview

Mission

One by one's mission is to help end obstetric fistula and improve the lives of women and girls suffering with this childbirth injury.

Major Activities

Activity 2
One by one believes that ending fistula will require the partnerships of governemnts, individuals, corporations, foundations along with other community-based organizations both international and in the united states. As part of our strategic plan to build a movement as a means to end fistula, one by one conducts numerous programs and activities to achieve this goal. One of our programs is a us-based youth program called young global leaders. Each year in the fall we accept middle and high school students from seattle-area schools to join us for our year-long advocacy training program. The program offers the opportunity to learn more about obstetric fistula and one by one's work to eradicate it, as well as women's and girls'issues worldwide, global development, and complex public health systems. This program has two beneficiary groups - the middle and high school youth and the college-aged interns who lead the program.student participants have the opportunity to use and develop their leadership skills through various presentations and projects around the community at their local schools.we also offer ways for our us-based young global leaders to connect to our youth program in kenya. In 2012 we had 11 students graduate from the program. Six students went to washington dc and met with four lawmakers to discuss fistula-related funding from congress. Additionally, to add to their learning about how funds are lobbied for and moved they met with us aid, the office of global women's issues, care, the un foundation, unfao, the former chief of staff for representative carolyn maloney of new york and participated in the v-day rally on feb. 14th. The college-aged interns both teach this program and benefit from a leadership track that offers them curriculum in leadership, teaching and public health to equip them to be the best teachers possible during this program as well as equipped at future leadership on issues about fistula. This program is in addition to our work with us-based partnerships that aid our work to end fistula, public speaking, university engagement and other educational work to build awareness and a movement to end fistula.
Activity 3
Bugando medical centerone by one pursues research opportunities when we believe that the results will directly inform our planning of impactful programs.the results of the research we continued to support in 2012 may very well lead to many more women delivering their babies safely. We completed enrollment of 396 women in our ground-breaking fistula prevention study at bugando medical center. In partnership with philips healthcare and sonosite, we have been evaluating whether ultrasound can be used to identify women in early pregnancy that are at high risk of experiencing obstructed labor and developing fistula. We are looking forward to results being ready in 2013! If ultrasound proves to be an effective screening tool, this technology will help ensure that high-risk women receive the care they need during labor to prevent fistula.the study enrolled its final patient in october 2011. Study authors have spent the end of 2012 and the first half of 2013 analyzing data and writing the manuscript. Study results are expected to be published in fall 2013 after careful analysis.

IRS990/Description0LET'S END FISTULA, A COMPREHENSIVE PROGRAM OF ONE BY ONE CURRENTLY OPERATING IN WESTERN KENYA, IS TACKLING THE PROBLEM OF OBSTETRIC FISTULA COMPREHENSIVELY. WITH OUR KENYAN TEAM ON THE GROUND WE HAVE DEVELOPED AN INNOVATIVE AND POWERFUL APPROACH TO CARRYING OUT THE EDUCATION AND OUTREACH THAT IS SO ESSENTIAL FOR FINDING WOMEN WITH FISTULA WHO ARE DEEPLY ISOLATED. OUR GOAL IS FOR LET'S END FISTULA TO SERVE AS A REPLICABLE MODEL FOR ENDING FISTULA IN OTHER REGIONS. LET'S END FISTULA HAS FIVE MAJOR PLANNED COMPONENTS WITH MEASURABLE ACTIVITIES AND RESULTS IN EACH CATEGORY. READ MORE ABOUT OUR ACCOMPLISHMENTS AND WORK IN 2012 ON SCHEDULE O.THE FIVE COMPONENTS ARE: OUTREACH/EDUCATION/TRANSPORTATION, SURGERY, REINTEGRATION, DATA COLLECTION AND MONITORING, AND PREVENTION. WE OFFER FREE TRANSPORT AND SURGICAL TREATMENT FOR EVERY FISTULA PATIENT OUR TEAM FINDS. ADDITIONALLY ANY PATIENT THAT COMES TO OUR PARTNER CLINIC, EVEN OUTSIDE OUR OUTREACH PROGRAM, IS REPAIRED FREE OF CHARGE AS WELL. WE PROVIDE REINTEGRATION SUPPORT SERVICES FOR WOMEN WHEN THEY RETURN TO THEIR COMMUNITIES FOLLOWING TREATMENT. WE ARE LAYING THE FOUNDATION FOR IMPLEMENTING PREVENTION ACTIVITIES THAT HAVE PROVEN TO BE EXTREMELY EFFECTIVE IN OTHER SETTINGS.WE WORK TO TACKLE THE PROBLEM OF OBSTETRIC FISTULA COLLABORATIVELY. WE KNOW THAT A SINGLE ORGANIZATION CANNOT SOLVE THIS PROBLEM. HOWEVER, DEDICATED ORGANIZATIONS WORKING TOGETHER - BRINGING EXPERTISE, RESOURCES AND FOCUSED DETERMINATION - CAN MAKE THIS DEVASTATING CHILDBIRTH INJURY A THING OF THE PAST. WE BELIEVE IN THE POWER OF PARTNERSHIP, AND LET'S END FISTULA IS A SHINING EXAMPLE OF HOW SMALL GRASS-ROOTS ORGANIZATIONS AND LARGE INTERNATIONAL CORPORATIONS (AND EVERYTHING IN-BETWEEN) CAN COME TOGETHER TO WORK TOWARD A COMMON GOAL OF ENDING FISTULA.WE BELIEVE IN TACKLING THE PROBLEM OF OBSTETRIC FISTULA WITH LOCAL LEADERSHIP. WE ARE WORKING WITH AN EXTRAORDINARY TEAM, ON THE GROUND IN KENYA THAT OVERSEES ALL ASPECTS OF THE LET'S END FISTULA PROGRAM. REGIONAL REPRESENTATIVES FROM RURAL VILLAGES THROUGHOUT WESTERN KENYA ARE PROVIDING VERY LOCAL LEADERSHIP TO OUR EFFORTS TO CREATE CONNECTION WITH THE MOST HARD TO-REACH WOMEN.OUR GOAL IS FOR LET'S END FISTULA TO SERVE AS A REPLICABLE MODEL FOR ENDING FISTULA IN OTHER REGIONS.OUTREACH, EDUCATION, TRANSPORTMOST WOMEN AND GIRLS WITH FISTULA LIVE IN ISOLATION IN REMOTE VILLAGES. THEY SUFFER IN SILENCE, SOMETIMES FOR AN ENTIRE LIFETIME. THEY HAVE LITTLE ACCESS TO INFORMATION ABOUT WHAT IS WRONG WITH THEM AND ARE OFTEN UNAWARE THAT FREE TREATMENT IS AVAILABLE. STRONG COMMUNITY-BASED EDUCATION AND OUTREACH IS ESSENTIAL IN ORDER TO FIND THESE WOMEN, BRING THEM OUT OF THE SHADOWS, AND CONNECT THEM WITH THE CARE THAT WILL TRANSFORM THEIR LIVES. IN 2012 OUR REGIONAL REPRESENTATIVE OUTREACH TEAM CONDUCTED 962 ACTIVITIES THAT REACHED 63,562 PEOPLE. OUR SPECIALIST TEAM SCREENED 1288 WOMEN TO DETERMINE IF THEY HAD FISTULA AND OFFICIALLY LISTED 405 OF THOSE WOMEN WITH ACTUAL FISTULA. WE PAID FOR THE ROUND TRIP TRANSPORT FOR 275 WOMEN TO OUR PARTNER SURGICAL FACILITY GYNOCARE, LOCATED IN ELDORET. ADDITIONALLY OUR THREE STAFF MEMBERS RECEIVED LEADERSHIP AND CAPACITY BUILDING TRAINING AND OUR 30 REGIONAL REPRESENTATIVES RECEIVED TWO CAPACITY BUILDING TRAININGS.FREE SURGICAL REPAIRONE BY ONE HAS PARTNERED WITH DR. HILLARY MABEYA, A HIGHLY REGARDED FISTULA SURGEON, TO ESTABLISH KENYA'S FIRST HEALTH FACILITY DEDICATED TO CARING FOR WOMEN WITH FISTULA. LOCATED IN ELDORET, GYNOCARE FISTULA CENTER PROVIDES COMPREHENSIVE FISTULA TREATMENT FOR WOMEN THROUGHOUT WESTERN KENYA, FREE OF CHARGE. WOMEN TYPICALLY SPEND TWO FULL WEEKS AT GYNOCARE. IN 2012 GYNOCARE, WITH ONE BY ONE'S FUNDING AND PARTNERSHIP GAVE FREE FISTULA SURGERY TO 216 OF THE WOMEN ON OUR WAITING LISTS.REINTEGRATIONONE BY ONE IS COMMITTED TO ENSURING THAT, ONCE A WOMAN'S FISTULA IS SURGICALLY REPAIRED, SHE HAS THE SUPPORT AND RESOURCES NEEDED TO SUCCESSFULLY RESUME A PRODUCTIVE LIFE. TO THAT END OF THE 300 WOMEN WE REPAIRED FROM 201

IRS990ScheduleF/Form990ScheduleFPartV/Explanation0SCHEDULE F, PART I, LINE 2: DATA COLLECTION AND MONITORING AND EVALUATION ONE BY ONE BELIEVES IN ROBUST DATA COLLECTION THAT WILL HELP US IMPROVE OUR PROGRAM ITERATIVELY AND NIMBLY. WE UTILIZE OUR COMMUNITY PARTNERS IN THE MOST EFFICIENT WAYS POSSIBLE TO GET TIMELY ACCURATE DATA ON ACTIVITIES AND OUTCOMES. WE HAVE DEVELOPED MONITORING AND EVALUATION INSTRUMENTS FOR EACH STAGE OF THE LET'S END FISTULA CONTINUUM. THEREFORE WE HAVE DATA COLLECTION FOR OUTREACH AND ACTIVITIES, PATIENTS FOUND, SCREENING ACCURACY RATES, TRANSPORT, CLINICAL DATA AND REINTEGRATION FOLLOW-UP DATA HAS JUST BEGUN TO BE COLLECTED. ALL OF THIS DATA IS AGGREGATED INTO A DATABASE AND CAN BE SEARCHED IN A RANGE OF WAYS. OUR CLINICAL DATA AND FOLLOW UP DATA IS BEING COLLECTED WITH A DIGITAL PEN THAT ALLOWS THE DATA TO BE UPLOADED TO A CLOUD DATABASE AS SOON AS THE PEN IS DOCKED EACH DAY OR WEEK. THE DATA COLLECTED BY THE REGIONAL REPS AND BY THE LET'S END FISTULA YOUTH CLUB IS COLLECTED ON PAPER AND SUBMITTED TO OUR DATA COORDINATOR EACH MONTH FOR ENTRY INTO OUR DATABASE. FOR THE FUNDS WE SEND TO CLINICAL PARTNERS TO PAY FOR SURGERIES WE HAVE A LIST OF WAITING PATIENTS THAT IS ASSEMBLED BY OUR REGIONAL REPRESENTATIVES THROUGH THEIR ACTIVITIES. THAT LIST IS CHECKED THROUGHOUT EACH MONTH BY OUR KENYA CONSULTING LEADERSHIP STAFF AND WOMEN ARE SENT IN FOR SURGICAL REPAIR AS SPACE BECOMES AVAILABLE AT OUR CLINICAL FACILITY AND BASED ON WHERE THE PATIENT IS ON THE LIST. THE ONLY PATIENTS THAT CAN JUMP THE LINE IN OUR WAITING LIST ARE: THE ELDERLY, THE VERY YOUNG AND WOMEN WHO HAVE FISTULA CAUSED BY RAPE. ONCE THE WOMEN ARRIVE AT OUR CLINICAL FACILITY WE CHECK THEIR NAMES AGAINST OUR WAITING LIST AND MARK THEM AS TRAVELED TO CARE. ONCE THE WOMEN ARE DISCHARGED FROM THE FACILITY POST-REPAIR THEY ARE MARKED AS DISCHARGED. WE THEN RECEIVE A LIST OF PATIENTS WHO RECEIVED SURGERY THE PREVIOUS MONTH AND WE CHECK THAT LIST AGAINST OUR PATIENT OUTREACH AND DISCHARGE LIST. ONCE THAT RECONCILIATION IS COMPLETE WE POST PAY FOR THE SURGERY FEES BY WIRING THE FUND AMOUNT THAT EQUALS THE COST PER SURGERY THAT WE HAVE AGREED TO WITH THE CLINICAL FACILITY TIMES THE NUMBER OF WOMEN REPAIRED THE PREVIOUS MONTH. FOR ALL OTHER PROGRAMMATIC EXPENSES, WE HAVE ACCOUNTING FORMS THAT ARE SUBMITTED BY EACH STAFF AND CONSULTING STAFF MONTHLY AND REVIEWED AND RECONCILED WITH RECEIPTS AND BANK STATEMENTS FIRST INTERNALLY BY OUR FINANCE TEAM AND THEN BY AN OUTSIDE BOOKKEEPER.
