GYNOCARE WILL SERVE A FIVE-COUNTY REGION IN WESTERN KENYA WITH A POPULATION OF OVER 4 MILLION PEOPLE. LACK OF ACCESS TO BASIC AND EMERGENCY OBSTETRIC CARE IN THIS AREA RESULTS IN ONE OF THE HIGHEST MATERNAL MORTALITY RATES IN THE WORLD AND A VERY HIGH INCIDENCE OF OBSTETRIC FISTULA. IN 2010, ONE BY ONE PURCHASED ESSENTIAL EQUIPMENT FOR GYNOCARE'S OPERATING THEATRE, AND SUPPORTED GYNOCARE'S FINAL PREPARATIONS FOR LAUNCHING THIS MUCH-NEEDED FISTULA REPAIR PROGRAM. OUR CONTRIBUTION INCLUDED WORKING CLOSELY WITH DIRECT RELIEF INTERNATIONAL TO FACILITATE A MOST GENEROUS DONATION OF ESSENTIAL SUPPLIES VALUED AT $42,000. GYNOCARE OPENED ITS DOORS IN THE SPRING OF 2011, AND EXPECTS TO CARE FOR AT LEAST 150 WOMEN IN ITS FIRST YEAR OF OPERATION. - 1 OPERATING TABLE - 25 BED FACILITY - $42K WHOLESALE VALUE OF SUPPLIESTANZANIAPERAMIHO MISSION HOSPITAL ONE BY ONE HAS BEGUN INVESTING IN CRITICAL OUTREACH EFFORTS TO FIND WOMEN WHO ARE LIVING WITH FISTULA IN REMOTE HARD-TO-REACH AREAS AND ARRANGE FOR THEM TO RECEIVE THE CARE THEY NEED. IN 2010, WE ENTERED INTO A PARTNERSHIP WITH PERAMIHO MISSION HOSPITAL IN SOUTHERN TANZANIA TO CARRY OUT INTENSIVE OUTREACH IN THREE RURAL DISTRICTS THAT ARE KNOWN TO HAVE HIGH RATES OF FISTULA. THE FISTULA TEAM AT PERAMIHO, HEADED BY DR. GAUDENS KOMBA, HAS DEVELOPED A HIGHLY EFFECTIVE APPROACH TO OUTREACH THAT INCLUDES ENGAGING COMMUNITY LEADERS TO SUPPORT EDUCATION EFFORTS, WORKING WITH COMMUNITIES TO EDUCATE ABOUT FISTULA AND THE AVAILABILITY OF FREE TREATMENT, SCREENING WOMEN IN LOCAL HEALTH CENTERS TO CONFIRM THAT THEY HAVE A FISTULA, AND ARRANGING FOR TRANSPORTATION TO PERAMIHO MISSION HOSPITAL FOR TREATMENT. DR. KOMBA HAS IDENTIFIED 102 WOMEN AND EXPECTS TO IDENTIFY 50 MORE IN THIS FIRST ROUND OF OUR OUTREACH PARTNERSHIP. THESE WOMEN, MANY OF WHOM HAVE LIVED WITH FISTULA FOR YEARS, RECEIVE LIFE-TRANSFORMING CARE AND RETURN TO THEIR COMMUNITIES ABLE TO RESUME PRODUCTIVE LIVES. - 102 PATIENTS - 3 DISTRICTSBUGANDO MEDICAL CENTERWE CONTINUED OUR LONG-TERM PARTNERSHIP WITH BUGANDO MEDICAL CENTER (BMC) IN 2010, SERVING THE POOREST AND MOST VULNERABLE WOMEN IN NORTHERN TANZANIA. ONE BY ONE'S MULTI-YEAR SUPPORT HAS ALLOWED BMC TO GROW AND SUSTAIN THEIR VITALLY IMPORTANT FISTULA REPAIR PROGRAM THAT NOW PROVIDES TREATMENT FOR 200 TO 300 WOMEN EACH YEAR, FREE OF CHARGE. IN NOVEMBER OF 2010, WE WERE HONORED TO HOST TWO EXTRAORDINARY WOMEN FROM BMC FOR A VISIT TO SEATTLE - YACINTA MUKAMA, A DEDICATED FISTULA NURSE AND LIZ MACH, A MATERNAL HEALTH NURSING EXPERT AND FORMER DIRECTOR OF BMC'S FISTULA PROGRAM. MANY PEOPLE IN THE ONE BY ONE COMMUNITY HAD AN OPPORTUNITY TO HEAR THESE INSPIRING WOMEN SPEAK ABOUT THEIR TIRELESS EFFORTS TO IMPROVE THE LIVES OF WOMEN WHO HAVE SUFFERED WITH FISTULA. - 208 REPAIR SURGERIESPREVENTIONETHIOPIAAFAR PASTORALIST DEVELOPMENT ASSOCIATION (APDA) WE CONTINUED TO SUPPORT THE TREMENDOUS EFFORTS OF APDA TO BRING VITAL MATERNITY SERVICES TO THE AFAR REGION OF ETHIOPIA. HOME TO APPROXIMATELY 1.3 MILLION MOSTLY PASTORALIST PEOPLE, THE AFAR REGION HAS ONE OF THE HIGHEST MATERNAL MORTALITY RATES IN THE WORLD AND A VERY HIGH INCIDENCE OF OBSTETRIC FISTULA. WITH SUPPORT FROM ONE BY ONE IN 2010, APDA TRAINED AND EQUIPPED TRADITIONAL BIRTH ATTENDANTS LEADING TO SAFER CHILDBIRTH FOR MANY WOMEN IN THIS REGION. ONE BY ONE ALSO PURCHASED A GENERATOR FOR THE VERY FIRST FACILITY IN THIS REGION CAPABLE OF DELIVERING EMERGENCY OBSTETRIC CARE. THIS WAS THE FINAL PIECE OF EQUIPMENT NEEDED BY THE BARBARA MAY MEMORIAL HOSPITAL BEFORE OPENING ITS DOORS IN 2011. WE ARE VERY GRATIFIED TO HAVE CONTRIBUTED TO BUILDING THE CAPACITY OF THIS TREMENDOUSLY IMPORTANT NEW FACILITY THAT WILL SAVE THE LIVES OF MOTHERS AND BABIES, AND PREVENT FISTULA. -

THE PROCESS STARTS WITH A LETTER OF INQUIRY THAT ASKS FOR THE DETAILS OF THE ORGANIZATION'S BUDGET, STAFFING AND PROGRAMS. THIS INQUIRY LETTER IS REVIEWED BY OUR GRANT MAKING COMMITTEE MADE UP OF BOARD MEMBERS AND EXPERT COMMUNITY MEMBERS. QUALIFIED ORGANIZATIONS THAT MEET OUR CRITERIA ARE INVITED TO SUBMIT A FULL PROPOSAL WHICH ASKS MORE DETAILED QUESTIONS ABOUT THE PROJECT FOR WHICH THEY DESIRE SUPPORT, STAFFING, BUDGET, TIMELINE, OUTCOMES, ETC. PROPOSALS ARE REVIEWED TWICE ANNUALLY BY THE GRANT MAKING COMMITTEE. A DOCKET IS PRESENTED TO THE FULL BOARD TWICE ANNUALLY AND THE FULL BOARD VOTES ON THE RECIPIENTS OF ONE BY ONE'S SUPPORT. ONCE SUPPORT IS CONFIRMED FUNDING IS GIVEN THROUGH WIRE TO AN APPROVED BANK ACCOUNT. THE WIRE IS MONITORED AND CONFIRMED BY ONE BY ONE AND THE GRANTEE. TECHNICAL SUPPORT IS ONGOING FROM ONE BY ONE IN THE FORM OF PROJECT DESIGN, COORDINATION, MONITORING AND EVALUATION. GRANTEES ARE EVALUATED TWICE DURING THEIR ANNUAL GRANT CYCLES - ONCE MIDYEAR AND ONCE AT THE END OF THEIR GRANT CYCLE. THE MONITORING FORMS ARE WRITTEN BY AN EPIDEMIOLOGIST AND REVIEWED BY THE GRANT MAKING COMMITTEE SO THAT ANY COURSE CORRECTIONS NECESSARY CAN BE MADE MIDYEAR. ONE BY ONE'S PROGRAM STAFF COMMUNICATE VERY REGULARLY WITH OUR GRANTEES TO MONITOR PROGRESS AGAINST GOALS AND TO OFFER TECHNICAL ASSISTANCE. STAFF VISIT GRANTEES ONCE PER YEAR FOR EVALUATIVE VISITS.
