Our mission is to provide or obtain services for people with developmental disabilities and their families in western new york, which will assist in achieving their highest level of independent living, enable their fullest participation in society, ensure that their civil and human rights are protected, and promote their ability to be productive, contributing members of the community.

The purpose of the conflict of interest policy is to ensure that the board of director's deliberations and decisions are made in the interests of community services for the developmentally disabled and to protect their interests when contemplating entering into a transaction, contract or other arrangement. The board of directors has a fiduciary duty which carries with it a broad and unbending duty of loyalty and fidelity. The interests of the agency must be the first priority in all decisions and actions. The board of directors have the responsibility to govern honestly and prudently and to exercise their best care, skill and judgment. They shall not use their positions or knowledge gained from their position for personal or financial benefit for themselves or for a third person including another nonprofit organization.

Form 990, Part VI, Section B, Line 15

It is the responsibility of the board of directors to ensure that an employment contract for the president and ceo is developed, approved and maintained in force. The board of directors are responsible to ensure compliance with the new york state executive compensation rules on covered providers, the internal revenue service guidelines for executive compensation and all other applicable laws and regulations. The board of directors are responsible to establish and provide a fair, competitive while not excessive compensation package for the president and ceo at the december board meeting each year for the coming year. The board may delegate the task of preparing a compensation package to the board chairperson and the board assessment and compensation committee. The chairperson of the board of directors serves as chair of the assessment and compensation committee.

Form 990, Part III, Program Services

Housing: community services for the developmentally disabled offers a variety of different housing options to meet ever person's needs. Our residences are inviting, comfortable homes that range from independent living options to houses with 24-hour staff assistance. Group-style iras have 24-hour on-site staff support. Apartment-style iras have two levels of on-site staff supervision, supported (staff on site day hours) or a supervised (staff on site 24/7) setting. Our emergency residence can provide temporary housing to individuals with developmental disabilities who are in need of a place to live. Emergency residence also provides site-based respite in an overnight capacity to people with developmental disabilities over the age of 18 years. All of our houses have access to a site van and many are wheelchair accessible. Community services offers a variety of low-income rental apartments to individuals of any ability. Amenities include a private bath, bedroom, kitchen and living room. There are also on-site laundry facilities and other options in some of the apartments. All apartments are located on major bus routes, making travel easy. Rent includes most utilities. Respite is the service most often requested by family caregivers. Our respite programs offer planned short-term and time-limited opportunities. A parent or guardian looking for a safe and positive place to care for a loved one for a specific time, they can choose from our after school respite, saturday respite or overnight respite. Medicaid service coordination: medicaid service coordinators (msc's) help develop goals, address needs, and explore individual's options through person centered planning. Msc's provide many services including financial, medical, and habilitation resources. Msc's can provide referrals to important benefits and links to a diverse menu of services. Parents and youth program specializes in meeting the unique needs of families and assists with obtaining or maintaining public benefits like medicaid, temporary assistance, ssi and ssd and links to services including community habilitation services, residential services, group day programs (including our expressive arts program), respite, and counseling. Parenting classes offer training for parents and soon-to-be parents. Our senior services program assist people and their families to understand the options available as they grow older. Adults age 50+ who have developmental disabilities will require special supports such as day programs, respite services, future life planning, healthcare services and residential placement. We place an emphasis on meeting medical needs and ensuring delivery of your senior benefits. Our service coordinators provide linkage to senior day programs, retirement programs, housing and medical services to give you and your family peace of mind. Our specialized case management provides services to youth and adults with developmental disabilities, who have a substance abuse history, are considered at-risk, or who are involved in the criminal justice system. Service coordinators develop a plan based on individual needs to prevent recidivism. We provide linkage to outside services including counseling and substance abuse treatment to keep you on a successful path. We provide advocacy and assistance throughout the court process and monitor legal obligations, working with probation officers and courts to establish a plan for success. Habilitation programs: community services offers three certified day habilitation site based programs for adults with developmental disabilities. Our certified programs are provided monday through friday at our 2180 genesee, 1377 abbott road, and 3784 commerce court locations. All programs offer recreation and ongoing volunteer opportunities. Transportation is provided to and from the facilities. Day habilitation programs focus on skills and activities that lead to increased independence in the home, school, job and community. Project coach is an after-hours telephone support hotline for individuals with developmental disabilities and their families. The hotline is available after 4 pm for support or assistance. Coach staff can provide information, referrals, outreach, and counseling and crisis intervention. They can also assist with medication reminders and do wellness check calls to prevent isolation and loneliness. Coach is a lifeline for you to stay connected in your community. Fill-in-the-gap transportation: the transportation services program is designed to assist those who have limited access to adequate transportation get to their desired destination. Those who are opwdd eligible and live with family may qualify for transportation to destinations on evenings, weekends and holidays, provided there are no other transportation options. For those who live in rural areas where transportation is generally limited, this program is available during daytime hours as well. Developmentally disabled offender program (ddop): community services has western new york's only dd offender program dedicated to helping individuals with developmental disabilities to navigate the various stages of the criminal justice system. This program provides specialized case management to individuals with developmental disabilities that are involved in the legal system or are at risk for becoming involved in the legal system. A service coordinator provides critical support to: develop an individualized treatment plan to reduce recidivism and increase opportunities for success. Make appropriate linkage with mental health therapy, chemical dependency treatment, vocational services, housing options, and financial services. Assist with advocacy for court, probation, and parole situations to ensure recommendations are followed and accurately reported.

PART IV, LINE 2B:

Csdd is a representative payee, as defined by the united states department of social security administration, for certain consumer funds. As a representative payee, csdd is responsible for assisting clients in managing their social security and supplementary security income benefits. Client funds are used for authorized and allowable purchases of items that enhance or improve the client s quality of life.

PART X, LINE 2:

It is highly certain that some positions taken for income tax purposes would be sustained upon examination by the taxing authorities, while others are subject to uncertainty about the merits of the position taken or the amount of the position that would be ultimately sustained. The benefit of a tax position is recognized in the consolidated financial statements only to the extent that an uncertain tax position (if any) is attributable to the companies. The tax returns for the tax years 2012 through 2015 remain subject to examination by the internal revenue service for federal tax purposes and also by nys for state tax purposes.
