North Country Management Corp was formed to provide technical, administrative, and support services. They provide the chapters with Medicaid and third party billing, accounts payable, payroll services, human resources, training and IT services.

The organizational structure caps the size of the board at one for each chapter, who receive a weighted vote of between 1 and 10 based on a published chart of the number of members.

NYSARC, Inc. is a unitary corporation consisting of its 45 Chapters. NYSARC, Inc. Essex County Chapter is one division of the corporation. The Corporation and Chapter's governing body is the Board of Governors representing the individual membership in each Chapter's jurisdiction. Each Chapter has one primary and one alternate governor with a weighted vote based on membership. The Corporation through its bylaws delegates day-to-day operating authority to the Chapter's Board of Directors. The Executive Committee of the Board of Governors (see Part VI, Line 7a below) comprises the Corporation's elected officers and exercises all powers of the Board of Governors between plenary meetings of the Governors.

The Board of Governors does not review Chapters' Form 990s before they are filed. Each chapter submits a copy of its Form 990 to the governing body when the Chapter's 990 is filed. Each Chapter must have a process for its Board of Directors to review the Form 990. The Essex County Chapter's process is as follows:The audited financial statements are reviewed by the entire Board of Directors at a regularly scheduled Board meeting. Prior to filing, Form 990 is provided to the entire Board of Directors and reviewed by them.

Form 990, Part VI, Line 12C: Explanation of Monitoring and Enforcement of Conflicts

Mountain Lake Services uses the following process to regularly and consistently monitor and enforce compliance with the policy:A) Education and Training.On a Monthly basis, the Corporate Compliance Officer holds Education and Training sessions on Corporate Ethics, Compliance, monitoring and the consequences of infringing the Agency compliance policy. The sessions are required for a) All Direct Support Professionals. b) Managers and Program Directors c) Consultants d) Board Members. B) Monitoring.On at least a quarterly basis, and if needed more frequently in an ad-hoc basis, the Corporate Compliance and Ethics Committee meets to discuss issues related to any and all conflict of interest involving staff, consultants and the Board. C) Enforcement1. Issues related to violation of corporate compliance policy is investigated promptly, thoroughly and Completely.2. Appropriate actions are taken against individuals, groups or entities, commensurate with the nature and gravity of non-compliance. D) Documentation.1. Signatures and dates of attendance by Staff, Consultants and the Board at meetings are available for review2. Minutes are recorded of all meetings and is available for review. 3. Report of investigations are available for review.

Comparability data is obtained by the Personnel and Finance Committee of the Board. The Executive Director's compensation is reviewed and approved by the entire Board of Directors on an annual basis.

Form 990, Part III, Line 4D: Other Program Services Description

OTHER PROGRAM SERVICES 4: EARLY INTERVENTIONThe Early Childhood Intervention services were started in 1980 to provide services to developmentally delayed infants 0 to 3 years of age. Emphasis is placed on providing training in the home to reduce the impact of the disability not only on the child, but on the family as well. Staff members go into the home to provide education and therapy to the child as well as training and support to the families. Other services are provided when needed. These include speech, physical and occupational therapy, family counseling, play therapy, parent training, advocacy and the coordination of medical and educational services. 79 persons benefited. OTHER PROGRAM SERVICES 5: RESIDENTIAL SERVICESMountain Lake Services operates Individual Residential Alternatives (IRA's). Each person living in an IRA is offered individualized support and opportunities in an environment where s/he can enjoy full participation in daily life, both at home and in the community. The residences are staffed as needed and training is provided in various daily living skills including cooking, shopping, managing money, personal hygiene and other skills to foster self-sufficiency. 210 Persons benefited from Residential Services. OTHER PROGRAM SERVICES 6: WORKCENTEREssex Industries is a vocational training center where people with disabilities learn the skills they need to work and live as independently as possible in the community. Since opening in 1974, Essex Industries has provided ongoing training on an increasing range of vocational experiences in an integrated setting. This training when coupled with social rehabilitation services guides individuals towards increased productivity and self-esteem. The workcenter's placement service locates jobs in competitive industry for ready trainees. Follow up services are then provided to ensure a successful transition to the workplace. 34 PERSONS BENEFITED. OTHER PROGRAM SERVICES 7: Article 16 ClinicMountain Lake Services operates Article 16 Clinics in Essex, St. Lawrence and Clinton Counties. These are outpatient clinics providing a variety of medical, diagnostic, and therapeutic services for persons with developmental disabilities and their families. 332 persons benefited. OTHER PROGRAM SERVICES 8: Fiscal IntermediaryMountain Lake Services offers Fiscal Intermediary services to individuals enrolled in Self-Directed Services. The Self-Directed model allows an individual, or their representatives, to hire their own employee(s). The individual then selects a Fiscal Intermediary Services Provider who is responsible for administrative and billing support. Mountain Lake Services receives a tiered flat rate for service fee from Medicaid. 124 persons benefited.
