Hospice of westchester is committed to providing compassionate end of life care. We combine the highest level of quality medical care with emotional and spiritual support enabling individuals and their loved ones to make the most of each day. The organization provides physical, psychosocial, medical and spiritual care wherever patients reside, including private homes, assisted living residences, nursing homes or hospitals. Working with primary care physicians, the organization provides a team of experts that includes a medical director, nurses, social workers, counselors, volunteers and home health aides. All staff are trained in compassionate end-of-life care and support.

“During 2012, the Organization developed a Pediatric Palliative Care Program known as Comfort Crew. Children ranging from newborn to age 21, who have been diagnosed with a progressive, life limiting illness, and who reside in Westchester County are eligible for services. A team of pediatric trained professionals will collaborate with the primary physician on a plan of care and provide pain and symtom management, social and psychological support, and spiritual and bereavement counseling to the child and family. Shereff Caregiver Program: The target population of the program represents individuals who are otherwise eligible for admission to Hospice & Palliative Care of Westchester but lack a primary caregiver who is able to provide the necessary care; and, who are not covered by private or governmental insurance programs that pay for such care; and, lack the financial resources to pay for a substitute caregiver. In circumstances where the Caregiver Needs Assessment Form identifies areas of patient need, Hospice & Palliative Care Of Westchester will arrange for and pay directly for an agency aide. In addition, Hospice & Palliative Care of Westchester will also include payment for a personal emergency response unit. The purpose of this program is to enable families to stay together in their home while on hospice and to avoid placement in a facility. The grant is most often used in circumstances where the caregiver is not well enough to care for the patient. The grant allows for care to be provided so that the patient's needs are attended to and the caregiver and patient can remain together at home. The Organization served 7 patients under the program during 2013. During 2012, the Caregiver Program was expanded to also support Complementary Care Services. |
