Leap, inc. Has its form 990 prepared by an outside accounting firm and has established the following review process to ensure that the information reported is complete and accurate. When the form 990 has been prepared, reviewed by management and is ready to be filed with the internal revenue service, it is electronically sent to the board members of the organization for any comments. Any comments are then grouped, summarized and provided to the outside accountants. Each issue is documented and addressed until the return is finalized and approved for filing.

Form 990, Part VI, Section B, Line 12C

All directors, employees and officers shall disclose any relevant interest that may pose a potential conflict to the organization upon election or appointment. Afterwards, the disclosure statements shall be updated annually. If an officer or director believes that they may have a potential conflict, full disclosure of all facts pertaining to such potential conflict shall be made to the board of directors. The board of directors of the corporation (or a duly appointed committee of the board) shall investigate the potential conflict of interest. The director or officer to whom the potential conflict of interest relates may offer factual information to the board or committee; but no such director or officer shall vote on such matter. The board or committee may, by majority vote, ask any such director or officer not to participate in any discussion relating to the conflict, or to leave the room in which such discussion is carried on; provided, however, that the interested director may participate in any discussion regarding his or her exclusion. Directors and officers to whom the potential conflict of interest relates shall not attempt to influence other directors regarding such matter, the board or committee shall determine by a majority vote of the disinterested directors whether the transaction or arrangement is in the corporation's best interest and for its own benefit and whether the transaction is fair and reasonable to the corporation and shall make its decision as to whether to enter into or allow the transaction or arrangement in conformity with such determination, the discussion on the potential conflict, and the vote thereon, shall be recorded in the minutes of the meeting of the board or committee.

Form 990, Part VI, Section B, Line 15A

The executive evaluation & compensation committee members, who do not have a conflict of interest, reviews information annually about the executive compensation of nonprofit peers as well as salary survey from independent sources. The executive evaluation & compensation committee makes a recommendation on ed compensation utilizing this information to the full board of directors who then votes on the annual compensation of the ed. The decision is documented in the minutes to the board. The operations and personnel committee reviews third party salary surveys and 990's of nonprofit peers and provides guidance to the ed on salary ranges for the ed to then finalize.

SO FAR, AMONG GRADUATES IN FISCAL 2019, 165 INDIVIDUALS HAVE SECURED THEIR NYS COMMERCIAL DRIVER'S LICENSE, AND RHOR HAS PLACED 98% OF LICENSED GRADUATES INTO JOBS WITH AN AVERAGE STARTING SALARY OF $18.85 PER HOUR.

IN FISCAL 2019, THE PROGRAM ENROLLED 85 INDIVIDUALS, GRADUATED 81% OF THEM AND PLACED 97% OF CERTIFIED GRADUATES IN JOBS WITH AN AVERAGE STARTING WAGE OF $20.36 PER HOUR."MADE IN NY" POST PRODUCTION TRAINING PROGRAM:THE "MADE IN NY" POST PRODUCTION TRAINING PROGRAM, LAUNCHED IN MAY 2017, PROVIDES PARTICIPANTS WITH THE CONTEXTUALIZED JOB READINESS, HANDS-ON SKILLS TRAINING, JOB PLACEMENT AND SUPPORT REQUIRED TO START A CAREER IN POST PRODUCTION. THE TRAINING LEADS TO CERTIFICATION BY THE NEW YORK CITY MAYOR'S OFFICE OF FILM, THEATRE AND BROADCASTING. TRAINING ALSO INCLUDES: JOB READINESS INCLUDING ON-CAMERA MOCK INTERVIEWS; JOB PLACEMENT ASSISTANCE; SOFT SKILLS; VOCATIONAL SUPPORT/COUNSELING; BENEFITS COUNSELING/SUPPORT AND COMPUTER LITERACY. IN FISCAL 2019, "MADE IN NY" POST PRODUCTION ENROLLED 45 INDIVIDUALS, WITH A GRADUATION RATE OF 96%. TO DATE, 72% OF GRADUATES HAVE BEEN PLACED IN POST PRODUCTION JOBS WITH INITIAL WAGES AVERAGING $19.49/HOUR.BROOKLYN NETWORKS:BROOKLYN NETWORKS IS BWI'S SIX-WEEK TELE-DATA CABLE INSTALLATION TRAINING PROGRAM. BROOKLYN NETWORKS TRAINS LOW-INCOME INDIVIDUALS FOR UPWARDLY-MOBILE JOBS INSTALLING AND MAINTAINING COMPUTER, VOICE, DATA, VIDEO AND SECURITY SYSTEM CABLING. UTILIZING A CUSTOM-BUILT TELECOMMUNICATIONS LAB LOCATED AT THE NEW YORK CITY COLLEGE OF TECHNOLOGY, THE PROGRAM PROVIDES TARGETED SKILLS TRAINING COMBINING THE INDUSTRY-ACCEPTED BUILDING INDUSTRY CONSULTING SERVICE INTERNATIONAL ("BICSI") CERTIFICATION WITH CUSTOMER SERVICE SKILLS, VOCATIONAL COUNSELING AND JOB READINESS TRAINING. IN FISCAL 2019, BROOKLYN NETWORKS ENROLLED 60 INDIVIDUALS, GRADUATED 90% OF THEM, AND SO FAR HAVE PLACED 94% OF BICSI-CERTIFIED GRADUATES IN JOBS WITH AN AVERAGE STARTING SALARY OF $16.76 PER HOUR.THE BROOKLYN WORKFORCE COLLABORATION:THE BROOKLYN WORKFORCE COLLABORATION IS A WORKFORCE DEVELOPMENT PARTNERSHIP THAT HELPS UNEMPLOYED AND LOW-INCOME NEW YORKERS ACCESS JOBS CREATED LOCALLY, PARTICULARLY WITH INDUSTRIAL BUSINESSES ALONG THE BROOKLYN WATERFRONT. BWI IS THE LEAD PARTNER AND IS PROVIDING EMPLOYER-CUSTOMIZED, RIGOROUS WORKFORCE DEVELOPMENT TRAINING, CAREER DEVELOPMENT AND SUPPORT SERVICES TO EMPLOYERS IN THIS GROWING AREA. THE COLLABORATION USES AN INTEGRATED STRATEGY OF EMPLOYER-CUSTOMIZED SOFT AND TECHNICAL SKILLS TRAINING, PRE-SCREENING AND ASSESSMENT; SECTOR-BASED SKILLS TRAINING; INCUMBENT WORKER TRAINING; LONG-TERM JOB PLACEMENT AND CAREER COUNSELING SUPPORT; AND ACCESS TO SOCIAL SUPPORTS AND RETENTION SERVICES. IN FISCAL 2019, BWI TRAINED 44 NEW YORKERS THROUGH THE INITIATIVE, GRADUATING 84% OF THEM AND SO FAR HAVE PLACEDG 70% OF GRADUATES INTO JOBS. AVERAGE STARTING WAGES FOR GRADUATES ARE $15.55 PER HOUR. NEW YORK DRIVES ("NYD"):NEW YORK DRIVES (NYD) IS BWI'S MULTI-SECTOR CREDENTIAL-BASED PROGRAM, WHICH PROVIDES CORE JOB-READINESS TRAINING AND ACCESS TO A NYS DRIVER'S LICENSE, WHICH MANY ENTRY-LEVEL JOBS AND SECTOR-FOCUSED TRAINING PROGRAMS REQUIRE. IN FISCAL 2019, NYD ENROLLED 39 INDIVIDUALS AND GRADUATED 35 (90%). TO DATE 28 GRADUATES HAVE SECURED THEIR DRIVER'S LICENSE AND 23 HAVE BEEN PLACED INTO A JOB (OTHERS ARE ACTIVELY TRAINING THROUGH A SUBSEQUENT BWI SKILLS TRAINING PROGRAM OR HAVE RECENTLY GRADUATED). STARTING WAGES FOR GRADUATES AVERAGE $20.91 PER HOUR.
