This policy is reviewed as the various contracts are reviewed, which is about annually and approved by the board of directors and key employee.

Form 990, Part III, Line 4D

Program Service Expenses 15,000, Grants and allocations 0, Revenue 0 Case Services - During the fiscal year ended 09/30/2018, there were over 32 different consumers that received financial support from Part B Case Service Funds and/or Restricted Funds received from donors that were used to assist these consumers in the removal or modification of barriers that will now allow these individuals with disabilities to live more independently in their homes or to perform their jobs more independently. Some examples of the assistance provided include, but are not limited to, aiding with transportation, food, clothing, utilities, housing security deposits, apartment holding fees, temporary PCA services, furniture as well as other assistance. Through the use of Community Block Grant Funding through the Town of Stratford Access Independence was able to construct two permanent metal ramps for individuals with disabilities living in the Town of Stratford. The constructed ramps will allow individuals to access the community for independent living and wellness purposes.

All of these services were provided to individuals with disabilities in order to provide assistance and promote a more independent life within their respective communities or jobs. During the fiscal year ended9/30/2018 there were 100 new or continued individuals served. Also, there were 3 individuals who received assistive technology and prothesis counseling and referrals 34 received independent living and life skills training, 13 had family services, counseling, education advocacy, peer counseling, personal care PCA and assistance referrals 4 had vocational referrals and coaching and transportation assistance and training 40 individuals received Housing, Home Modifications and Shelter Services. Finally, 981 received general info. referral sevices.

IRS990/ProgSrvcAccomActy2Grp/Desc0Nursing Facility Transition-Individuals with disabilities are assisted in achieving their chosen goals and objectives to transition out of a nursing facility into the community. In order to assist these individuals in their transition process, community services, resources and supports are identified and coordinated on behalf of these transitioning consumers in order to assist them in achieving their goal to live in the community. Advocacy methods are utilized when appropriate. There were 59 consumers assisted in this process with 15 successfully being transitioned out of the nursing facility. These individuals, now living in the community, in their residence of choice, receive 6 months of regular status follow-up checks, satisfaction surveys, and referral for any needed changes in their personal care plans. The agency continues their care coordination with other service partners. Additionally, all nursing facility transition consumers, whether transferred to the community, in process of transition, or remaining in the institution, are eligible for additional individual services

Through ADRC - MIPPA, more than 30 different presentations were made through the State of CT demonstrating ADRC and the related services as well as hundreds of phone and mailed contacts informing individuals about the Medicare Savings Program.

IRS990ScheduleO/SupplementalInformationDetail/ExplanationTxt0Program Service Expenses 15,000, Grants and allocations 0, Revenue 0 Case Services - During the fiscal year ended 09/30/2018, there were over 32 different consumers that received financial support from Part B Case Service Funds and/or Restricted Funds received from donors that were used to assist these consumers in the removal or modification of barriers that will now allow these individuals with disabilities to live more independently in their homes or to perform their jobs more independently. Some examples of the assistance provided include, but are not limited to, aiding with transportation, food, clothing, utilities, housing security deposits, apartment holding fees, temporary PCA services, furniture as well as other assistance. Through the use of Community Block Grant Funding through the Town of Stratford Access Independence was able to construct two permanent metal ramps for individuals with disabilities living in the Town of Stratford. The constructed ramps will allow individuals to access the community for independent living and wellness purposes.
