Mission

Work with individuals with disabilities to enable them to live or work more independently in their communities and workplace. See Schedule O for a more detailed explanation of the Mission Statement.

The Disability Resource Center of Fairfield County, Inc. works with individuals with disabilities to enable them to live more independently in their communities. To achieve this objective, the organization provides advocacy, peer support, independent living skills training and provide information and referral services to these individuals with disabilities and/or their families who live primarily in the Southwestern Region of Connecticut and beyond.

The Conflict of Interest Policy is reviewed and updated periodically. Since a Conflict of Interest statement is required to accompany almost all contracts. This policy is reviewed as the various contracts are reviewed, which is about annually and approved by the board of directors and key employee.

The State of Connecticut has five 5 Centers for Independent Living with similar operations, which affords the Board of Directors to review comparable compensation data from the other four 4 Centers for Independent Living within CT who have similar operations and employees in the same exact position.

IRS990/Desc0Core Services - People with differing disabilities were provided assistance in various ways, including, but not limited to, providing peer counseling, independent living skills training, individual advocacy training, provided information and referrals, assisted with technology and assisted with transportation issues. All of these services were provided to individuals with disabilities in order to assist and promote a more independent life within their respective communities or jobs. During the fiscal year ended September 30, 2014 there were 256 new or continued individuals served. Also, there were 30 individuals who received assistive technology and prothesis counseling and referrals. Also, 25 received independent living and life skills training, 92 had family services, counseling, education advocacy, peer counseling, personal care PCA and assistance referrals 39 had vocational referrals and coaching and transpostation assistance and training. Finally there were 1,077 general information and referrals taken during the year.

IRS990/ProgSrvcAccomActy2Grp/Desc0Nursing Facility Transition - Individuals with disabilities are assisted in achieving their chosen goals and objectives to transition out of a nursing facility into the community. In order to assist these individuals in their transition process, community services, resources and supports are identified and coordinated on behalf of these transitioning consumers in order to assist them in achieving their goal to live in the community. Advocacy methods are utilized when appropriate. There was approx. 40 consumers assisted in this process with 20 successfully being transitioned out of the nursing facility. These individuals, now living in the community, in their residence of choice, receive 6 months of regular status follow-up checks, satisfaction surveys, and referral for any needed changes in their personal care plans. The agency continues their care coordination with other service partners. Additionally, all nursing facility transition consumers, whether transferred to the community, in process of transition, or remaining in the institution, are eligible for additional indivi

IRS990/ProgSrvcAccomActy3Grp/Desc0Case Services - During the fiscal year ended September 30, 2014, there were over 30 different consumers that received financial support from Part B Case Service Funds and/or Restricted Funds received from donors that were used to assist these consumers in the removal or modification of barriers that will now allow these individuals with disabilities to live more independently in their homes or to perform their jobs more independently. Such modifications may include ramps, bathroom and accessibility modification, as well as less traditional, but equally important, in order to keep the child safe at home purchase hearing aides so that hearing impaired individuals can continue to live with and communicate with their family assisted with repairs to consumer owned handicapped adapted vans and electric wheelchairs to allow the consumer throughout the community with less difficulty.
