Mission and Program Overview

Mission

Fellowship place's mission is to serve adults with mental illness by offering a full range of therapeutic support and rehabilitation services that promote independence, wellness, and a meaningful life. (see schedule o for continuations) services are provided on a multi-purpose campus where clients may access a variety of services in one location, open 365 days a year. All services complement traditional psychiatric care by providing clients opportunities for daily structure and a chance to learn and practice life skills, and form positive connections with peers and the community. Programs include daily meals, drop-in services, supportive education, supportive employment, supportive housing, clinical groups, physical recreation, expressive arts, case management, and supportive counseling.

The form 990 is completed and reviewed by the business director and the executive director. The form 990 is then forwarded to the board of directors.

Form 990, Part VI, Section B, Line 12C

The conflict of interest policy is reviewed by the board of directors annually. As part of the review process, officers, directors and trustees are required to disclose in writing any business or personal relationships which may present a conflict of interest. The president of the board of directors is responsible for reviewing all disclosure statements and reporting any potential areas of concern to the full board and the executive director. Any director having a duality of interest or possible conflict of interest in a matter before the board of directors must recuse him/herself from voting on the subject.

The executive director is reviewed annually by the executive committee. Decisions on salary adjustments are made by the president in collaboration with the finance committee. All other key employees receive periodic reviews by their direct supervisor. The board of directors adopts a budget for salary increases as part of its annual budget preparation.

AS A RESULT, REFERRALS WERE PLACED ON A WAITING LIST AND ADMITTED TO THE PROGRAM AS OPENINGS BECAME AVAILABLE. DESPITE THESE CHALLENGES, THE EMPLOYMENT PROGRAM CONTINUED TO HAVE EXCELLENT EMPLOYMENT RATES: 49% OF CLIENTS ENROLLED FOUND COMPETITIVE JOBS AND 94% OF EMPLOYED CLIENTS MAINTAINED THEIR EMPLOYMENT FOR 90 DAYS OR MORE. (SEE SCHEDULE O FOR CONTINUATION)IN JULY 2017 THE AGENCY WAS SELECTED TO BECOME PART OF A FEDERALLY FUNDED DEMONSTRATION PROJECT THAT PROVIDES SUPPORTED EMPLOYMENT SERVICES TO ADULTS WITH A HISTORY OF CRIMINAL JUSTICE INVOLVEMENT AND MENTAL ILLNESS. THIS PROJECT SERVED A TOTAL OF 44 CLIENT IN ITS FIRST YEAR OF OPERATION AND 25% OF CLIENTS ENROLLED WERE COMPETITIVELY EMPLOYED. THE PSYCHOSOCIAL REHAB CENTER SERVED 470 INDIVIDUALS WITH ATTENDANCE RANGING FROM 13 ON THE WEEKENDS TO 127 WEEKDAYS. THE CENTER DEVELOPED A NEW OLDER ADULTS GROUP FOR CLIENTS AGE 55 AND OVER. ABOUT 25% OF AGENCY CLIENTS ARE BETWEEN THE AGES OF 55 AND 75, AND THE GROUP ADDRESSES THEIR SPECIAL CONCERNS RELATED TO HEALTH CARE, INSURANCE, AND AGING IN PLACE. OTHER SPECIALTY GROUPS INCLUDED YOUNG ADULTS, PEER SUPPORT FOR SPANISH SPEAKING ADULTS EXPERIENCING MENTAL ILLNESS, AND SAFE SPACE FOR MEMBERS OF THE LGBT COMMUNITY. THE SUPPORTED EDUCATION PROGRAM SERVED 53 CLIENTS, OF WHOM 89% ENROLLED IN A POST SECONDARY EDUCATION PROGRAM.
