Mission and Program Overview

Mission

Fellowship place's mission is to serve adults with mental illness by offering a full range of therapeutic support and rehabilitation services that promote independence, wellness, and a meaningful life. (see schedule o for continuations) services are provided on a multi-purpose campus where clients may access a variety of services in one location, open 365 days a year. All services complement traditional psychiatric care by providing clients opportunities for daily structure and a chance to learn and practice life skills, and form positive connections with peers and the community. Programs include daily meals, drop-in services, supportive education, supportive employment, supportive housing, clinical groups, physical recreation, expressive arts, case management, and supportive counseling.

Governance Explanations

The form 990 is completed and reviewed by the business director and the executive director. The form 990 then forwarded to the board of directors.

Form 990, Part VI, Section B, Line 12C

The conflict of interest policy is reviewed by the board of directors annually. As part of the review process, officers, directors and trustees are required to disclose in writing any business or personal relationships which may present a conflict of interest. The president of the board of directors is responsible for reviewing all disclosure statements and reporting any potential areas of concern to the full board and the executive director. Any director having a duality of interest or possible conflict of interest in a matter before the board of directors must recuse him/herself from voting on the subject.

The executive director is reviewed annually by the executive committee. Decisions on salary adjustments are made by the president in collaboration with the finance committee. All other key employees are reviewed annually by their direct supervisor. The board of directors adopts a budget for salary increases as part of its annual budget preparation.

THE MAJORITY OF CLIENTS ARE SERVED IN THE PSYCHOSOCIAL REHAB CENTER, WHICH EXPERIENCED DAILY ATTENDANCE RANGING FROM 10 ON THE WEEKENDS TO 135 ON WEEKDAYS. THE CENTER PROVIDED APPROXIMATELY 32,000 FREE MEALS TO PROGRAM PARTICIPANTS. IN ADDITION TO REGULAR PROGRAMMING, THE CENTER INITIATED SEVERAL NEW GROUPS TO MEET CLIENTS' CHANGING NEEDS: SAFE SPACE FOR MEMBERS OF THE LGBT COMMUNITY, HEARING VOICES FOR CLIENTS COPING WITH AUDITORY HALLUCINATIONS, AND A WOMEN EMPOWERING WOMEN GROUP TO ADDRESS SELF-IMAGE AND SELF ESTEEM ISSUES EXPERIENCED BY MANY FEMALE CLIENTS. THIS YEAR THE STATE OF CT COMPLETED FIDELITY REVIEWS OF THE SUPPORTED EMPLOYMENT PROGRAM AND THE SUPPORTED HOUSING PROGRAM. THE PURPOSE OF THE REVIEWS IS TO DETERMINE HOW CLOSELY A PROGRAM IS FOLLOWING BEST PRACTICES AND MEETING SERVICE BENCHMARKS. SUPPORTED EMPLOYMENT WAS RATED "EXEMPLARY" WITH A SCORE OF 121 OUT OF A POSSIBLE 125. SUPPORTED HOUSING WAS RATED "HIGH QUALITY" WITH A SCORE OF 120 OUT OF A POSSIBLE 140. FIFTY-NINE% OF CLIENTS ENROLLED IN THE SUPPORTED EMPLOYMENT PROGRAM WERE COMPETITIVELY EMPLOYED. THE GOAL IS 35%. CASEWORKERS IN SUPPORTED HOUSING PROVIDED 2,400 SERVICE HOURS TO 45 RESIDENTS SPREAD ACROSS FOUR APARTMENT BUILDINGS. A MAJORITY OF RESIDENTS ARE FORMERLY HOMELESS INDIVIDUALS WHO FACE MENTAL HEALTH PROBLEMS AND OTHER SIGNIFICANT HEALTH ISSUES THAT MAKE THEM VULNERABLE AND WITHOUT SUPPORT SERVICES, MANY WOULD REQUIRE HIGHER LEVELS OF CARE. THE FELLOWSHIP INN, THE AGENCY'S HOMELESS DROP-IN CENTER, SERVED 131 UNDUPLICATED CLIENTS, A 32% INCREASE OVER LAST YEAR. DAILY ATTENDANCE IN THE INN RANGED FROM A LOW OF 17 TO A HIGH OF 42.
