“Form 990 is prepared by the auditors in conjunction with the accounting manager and the cfo. A copy of form 990 is distributed to the board of directors prior to filing the document with the internal revenue service.”
“The policy which addresses conflict of interest (policy # 101.1 ethics) is reviewed and approved annually by the board of directors. The agency communicates the ethics policy to all personnel and persons served. All personnel are oriented to the ethics policy at time of hire and in addition it is included in the staff handbook, which is signed by personnel that they have received, read and agree to abide by the contents of this policy. There is also a code of ethics book that is kept in the first floor information center from the various professional disciplines. It is also posted in the agency's waiting rooms for consumers to review and at the staff apartments in our residential sites. This policy is also incorporated into contracts with any vendors/suppliers. The policy's intent and its implications are addressed at staff and board meetings and in other clinical settings, as appropriate. In addition, we require that each employee take an annual e-learning course on compliance and ethics which includes material on conflict of interest. All staff are encouraged to report any potential conflict of interest as an adverse incident, which is then reviewed by the compliance officer and quality counsel for evaluation and feedback. Each supervisor is expected to follow up with employees to complete resolution of any incident. Additionally, the board of directors has adopted within the bylaws of bridges certain policies and procedures specific to the board regarding conflict of interest. Each board member is required to annually sign a statement which affirms that he or she has received a copy of the conflict of interest policy, has read and understands the policy, has agreed to comply with the policy, and understands the organization is charitable and in order to maintain its federal tax exemption it must engage primarily in activities which accomplish one or more of its tax exempt purposes.”
“As stipulated in the bylaws of the corporation, the process for determining compensation of the president/ceo includes a performance review of this individual by the executive committee of the board of directors. This process includes the review of specific performance elements, the comparing of data on compensation of other ceos as available through our association's annual salary survey, and open discussion among the members of this committee. The meeting is documented in minutes of the executive committee of the board of directors. The executive committee then makes a recommendation to the full board of directors regarding the annual compensation for the ceo for their approval.”

THE ADDICTIONS SERVICES PROGRAM IS THE ONLY COMMUNITY-BASED, NOT-FOR-PROFIT OUTPATIENT ADDICTION PROGRAM IN CATCHMENT AREA #6 AND THUS PLAYS AN IMPORTANT ROLE IN PROVIDING COMMUNITY-BASED OUTPATIENT TREATMENT FOR INDIVIDUALS WITH SUBSTANCE ABUSE PROBLEMS AND THEIR SIGNIFICANT OTHERS, THE GOAL OF THE PROGRAM IS TO PROMOTE RECOVERY AND IMPROVE THE QUALITY OF LIFE OF PERSONS WITH BEHAVIORAL HEALTH AND SUBSTANCE USE DISORDERS BY PROVIDING RESOURCES FOR UNDERSTANDING AND MANAGING A DISORDER. |

THE PROGRAM IS COMMITTED TO WORKING WITH THE CHILD'S ENTIRE ENVIRONMENT AND IS DESIGNED TO PROMOTE THE SOCIAL AND EMOTIONAL WELL BEING OF CHILDREN AND THEIR FAMILIES. THE GOAL IS TO ENHANCE THE ABILITY OF THE FAMILY TO REMAIN INTACT AND SUPPORT THE SAFETY AND WELL BEING OF THE CHILD IN THE LEAST RESTRICTIVE TREATMENT ENVIRONMENT ALL SERVICES ARE PROVIDED IN PARTNERSHIP WITH CHILDREN AND FAMILIES, BY ENCOURAGING PARTICIPATION IN SERVICE PLANNING, MAINTAINING A RECOVERY MODEL, AND PROVIDING SERVICES THATARE CULTURALLY COMPETENT. FAMILIES MAY INCLUDE CHILDREN AND THEIR BIOLOGICAL, FOSTER, ADOPTIVE PARENTS OR SIGNIFICANT ADULTS WITH WHOM THEY LIVE. FAMILY REUNIFICATION - THE INTENSIVE FAMILY PRESERVATION PROGRAM (PART OF THE FAMILY AND CHILDREN SERVICE) PROVIDES HOME-BASED SEVICES TO DCF FAMILIES AT RISK OF FAMILY DISRUPTION, OR HAVING A CHILD RETURNED FROM AN OUT OF HOME PLACEMENT DEPENDING UPON THE TYPE OF SERVICE (PRESERVATION OR REUNIFICATION), THE PROGRAM ACCEPTS REFERRALS FROM UP TO 22 TOWNS IN THE SOUTHWESTERN PART OF CONNECTICUT. THE CLINICIANS MEET IN THE FAMILY HOME FOR AN AVERAGE OF TWELVE WEEKS, SCHEDULING MEETINGS FOR TWO OR THREE TIMES PER WEEK. THE PROGRAM OPERATES MONDAY THROUGH FRIDAY, WITH EVENING AND WEEKEND TIMES AVAILABLE AS NEEDED. THERE IS A CLINICIAN ON CALL 24 HOURS A DAY, SEVEN DAYS PER WEEK. PSYCHIATRIC SERVICES- THE IICAPS PROGRAM PROVIDES INTENSIVE HOME-BASED SERVICES TO CHILDREN WHO ARE AT IMMINENT RISK OF OUT-OF-HOME PLACEMENT DUE TO MENTAL HEALTH ISSUES, BEHAVIORAL OR EMOTIONAL DISTURBANCES. THESE CHILDREN MAY HAVE RETURNED OR ARE RETURNING HOME FROM OUT-OF-HOME CARE AND REQUIRE A LESS INTENSIVE LEVEL OF TREATMENT. THE GOAL IS TO PROVIDE A DEGREE OF CLINICAL CARE AND SUPERVISION IN THE HOME AND COMMUNITY SETTING THAT IS APPROPRIATE TO CHILDREN DISCHARGED FROM A MORE RESTRICTIVE SETTING. ALL TREATMENT, CARE AND SUPPORT SERVICES MUST BE PROVIDED, IN A CONTEXT THAT IS CHILD-CENTERED, FAMILY-FOCUSED, STRENGTH-BASED, CULTURALLY COMPETENT AND RESPONSIVE TO EACH CHILD'S PSYCHOSOCIAL, DEVELOPMENTAL, AND TREATMENT NEEDS.CARE COORDINATION-EMS AND CARE COORDINATION ARE LINKED UNDER DCF'S KID CARE INITIATIVE TO PROVIDE IMMEDIATE AND COMPREHENSIVE ASSESSMENT CRISIS, STABILIZATION, DE-ESCALATION, AND REFERRAL SERVICES FOR CHILDREN IN CRISIS IN A NATURAL COMMUNITY SETTING. |
