Form 990 governing body review Part VI line 11

The original Form 990 is reviewed by the Executive Director with the preparer. Necessary adjustments are made, if applicable, a copy of the Form 990 is then provided to each governing board member for their review. Any questions or requests for information are returned to the preparer for response within the designated time frame. The board chairman then signs the Form 990 prior to mailing by the executive director.

Conflict of interest policy compliance Part VI line 12C

Each board member (including the Board Chairman and Executive Director) is required to annually complete a Conflict of Interest Questionnaire to insure that no conflict of interests exist. Any potential conflicts must be discussed and resolved prior to that member continuing to serve in their position on the governing board.

CEO executive director top management comp Part VI line 15A

The Executive Director receives an annual performance review by a committee of the governing board. This committee reviews the performance of the overall organization, as well as the executive directors annual goals and objectives. A merit increase and/or cost of living increase may be awarded based upon the review. During this process, the committee compares and reviews the compensation package of teh Executive Director with similar positions in like organizations within the community.

There are no other compensated officers of the organization. Key employee annual performance review is completed by Executive Director. During this process, the executive director compares and reviews the compensation package of the key employees in similar positions in like organizations within the community.

Governing documents etc available to public Part VI line 19

The governing documents, conflict of interest policy and financial statements of the Samaritan House Community Center are available for public inspection upon written request to the Executive Director. The Executive Director will contact the person making the request to schedule an appointment for the person to review the governing documents onsite. The conflict of interest policy and audited financial statments will be copied and mailed to the person making the written request.
