Annually, each member of the board is provided a conflict of interest questionnaire form to be completed, signifying that they obtained a copy of the policy, have read and comprehend the policy, and agree to comply with the policy. The board reviews and monitors any conflicts disclosed. All members of the board, trustees and officers that are making purchases or future related party transactions, must execute a conflict of interest disclosure form.
