Activity 2

Youth Enrichment Services1) Na Ohana Pulama Group and Foster Homes served emotionally and behaviorally challenged youth between the ages of 5 and 19 through therapeutic, non-institutional family environments on Oahu and the Big Island.2) Statewide Resource Families Program (SRF) served child-specific foster/resource families statewide. SRF provides an integrated and community-based approach to assess, train, and license child-specific foster/resource families (Special Licensed Relatives and Special Licensed Homes).3) Horizons assisted young adults transitioning to independent living through housing, personal assistance, vocational and educational support, and achievement of goals in the areas of independent living, education, and employment. Services are administered through weekly home meetings, quarterly reviews, and regular daily contact.

Activity 3

Community and Senior Services1)Maili Land Transitional Housing Program assisted individuals on Oahu and Kawaihae Transitional Housing Program assisted individuals on the Big Island with Housing Services focused on families with children under the age of 19 who were homeless. Services including transitional housing, case management, employment training, budgeting, and education classes were offered to help families become self-sufficient and obtain affordable permanent housing.2) General Immigration Services helped immigrants learn English and apply for citizenship and naturalization on Oahu and the Big Island.3) Employment Core Services provided immigrants with job skills and English training.4) Case Management provided seniors with advocacy, case management, professional counseling for psychological and social problems, assessment of needs, and bereavement counseling through Oahu senior housing projects and parishes.5) Comprehensive Individualized Services provided seniors with housekeeping, shopping, escort, translation, letter reading/writing, and para-professional counseling assistance. In addition, seniors received para-professional counseling for respite services or were linked with respite services for their care-giving responsibilities.6) Housing Assistance Program helped seniors on Oahu with housing crises, eviction prevention, counseling, and placement into affordable housing.7) Lanakila Multi-Purpose Senior Center kept seniors active, healthy, and connected to the community through educational, recreational, and social activities.8) Financial Advocacy Services for seniors assisted seniors on Maui and in managing their finances through budgeting, bill paying, and bill sorting services.9) Quality Living Choices provided seniors in foster homes on the Big Island with case management for their long-term care. These seniors would have otherwise been placed in a nursing home facility to meet their intense medical needs.10) Transportation Services drove seniors to doctor appointments, shopping, and group dining sites throughout the Island of Oahu.11) Developmentally Disabled Waiver Services provided personal care, skilled nursing, respite services, and overall nursing supervision to adults and children with developmental disabilities who reside in special foster homes or in their own homes.12) Medication Management provided clients with information and help managing their medication.
