“Pt III, Line 1, Description of Organization Mission: to foster the development of responsible community members, and to advocate for their needs in an effort to preserve family unity by providing: 24-hour crisis intervention services, temporary safe refuge during family conflicts and abuse, supportive counseling for youth and families, outreach, education and prevention programs.”

“Pt III, Line 4a, Program Service Accomplishments: Sagan Na Homlo Drug and Alcohol Rehabilitation Facility: Based on the therapeutic community model, the program provides residential treatment services for up to 8 youth between the ages of 12 and 17 who meet ASAM Level III.5 Placement Criteria. Youth over the age of 17 who are under the continuing jurisdiction of juvenile drug court are also eligible for the program. Length of program is determined by individual need but the average length of stay is 6-9 months. Services provided include individual, family and group counseling, recreation therapy, educational services and drug and alcohol treatment. Aftercare services are offered for youth and their families for up to one year to provide consistency and continued support. The program also works with collaborating agencies to provide a full continuum of care. 7 youth were served during FY 2023.”

“The fiscal officer reviews for accuracy the draft of Form 990 prepared by the paid preparer. The fiscal officer communicates with the paid preparer if there are any questions. In the past the governing body has not been provided a copy before the return is filed.”

“Financial statements consisting of a balance sheet and income statement are published annually in the local newspaper. In addition to the published financial statements, governing documents, conflict of interest policy and financial statements are available at the administration offices of Sanctuary. Upon request the documents can be made available for review. Requests for documents should be directed to the executive director's office.”
