Form 990, Part VI, Section B, line 11

The 990 is completed by an independent accountant and first reviewed by the Executive Manager with the Vice President of the Board, who is a CPA by profession, to ensure accuracy. The 990 is then reviewed and signed by the Board president. It is then discussed with the members of the Board.

Form 990, Part VI, Section B, line 12C

The Board of Directors meets quarterly with staff to monitor and enforce compliance policies at the staff and board levels. Minutes of its meetings are recorded and distributed to all members for review and retention. Board members regularly attend staff meetings and the Chairman of the Board is open to all employees, residents and volunteers for information or complaints about the organization and its staff.

Page 6, Part VI, Question 15a - The board of directors, each an independent person, reviewed, discussed and approved candidates and compensation for the Administrator at several board meetings which are documented in the minutes of the meeting. This included a discussion of salary ranges as well as candidate ability and experience, as well as the organizations financial ability for compensation. Page 6, Part Vi, 15b - There are no other key employees or paid officers.

IRS990/Desc0Mcintyre House is a long term residential substance abuse recovery program with 16 beds. McIntyre House provides structured services for about 70 residents per year, who stay in the house up to one year and are offered full residential and recovery services. The program is licensed by the Department of Alcohol and Drug Programs. Program activities include educational, therapeutic and support group activities as well as individual case management. The McIntyre House primary behavioral outcome objective is that 80% of all residents who complete the program (9 months or more) will fully abstain from all non-prescribed psychoactive drugs and alcohol. Other outcomes include improved and stable physical and mental health; abstinence from criminal and antisocial behavior; permanent housing; full time employment; decrease or absence of high-risk sexual behaviors; routine attendance at outside support groups; and routine participation in volunteer work or community service activities.
