Form 990, Part VI, Section A, Line 2

Jennifer giacalone, a board member, is the daughter of mary jo hazard, our current board president.

Form 990, Part VI, Section A, Line 8B

Minutes are kept of every full board meeting.

Form 990, Part VI, Section B, Line 11

The board of directors finance committee members review the tax return with the director of finance, executive director, and auditing firm representative. The return is then presented for the approval of the entire board of directors by the members of the finance committee.

Form 990, Part VI, Section B, Line 12C

The monitoring and enforcement of the conflict of interest policy is achieved by discussion at board meetings throughout the year and self disclosure

Form 990, Part VI, Section B, Line 15

To review and formulate all salaries and changes in salaries rainbow services uses a salary survey provided annually by the center for non-profit management. The survey is updated annually by the center for non profit management, and made available for purchase in the spring every year. Rainbow management reviews salary ranges and individual salaries each year during our budgeting process using the latest published survey as a benchmark. The executive director's salary is reviewed by the executive committee of the board of directors every january, while preparing for the executive director's annual date of hire anniversary review in february. The most recent salary survey is used as a benchmark within this review process, along with performance and other goals that had been set by the board. The executive committee makes recommendations to the full board of directors once the review is accomplished, and the full board votes on any changes recommended.
