Governance Explanations

Form 990, Part VI, Line 11B: Form 990 Review Process

The Board of Directors Operation/Finance Committee reviews the Form 990 before the 990 is signed by the Executive Director and reports to the full Board.

Form 990, Part VI, Line 12C: Explanation of Monitoring and Enforcement of Conflicts

The policies are reviewed annually with employees, therefore disclosure would be made annually if needed.

Form 990, Part VI, Line 15A: Compensation Review & Approval Process - CEO, Top Management

Compensation for the Executive Director is determined as part of their performance review conducted by the Governance Committee of the board each year. That review includes a board survey, a 360 degree review by staff and a salary survey that is conducted by our PEO (Insperity, Inc.). The salary survey obtains comparable data from similar nonprofit organizations. Salary increases are based on performance and fair compensation determined by the size of the organization.

Form 990, Part VI, Line 15B: Compensation Review and Approval Process for Officers and Key Employees

Haven Hills uses the Center for Nonprofit Management compensation & benefits survey, the region's only nonprofit salary data resource, as a guide to determine the compensation of our key employees. Our Board of Directors receive no compensation. Th Board of Directors reviews and approves all compensation to key employees. Any salary adjustments for key personnel are deliberated at Board meetings during the budget approval process and are taken to a Board vote.

Form 990, Part VI, Line 19: Other Organization Documents Publicly Available

The organization makes the governing documents, conflict of interest policy, and financial statements availble to the public upon request.

Additional Explanations

Form 990, Part III, Line 4A

The crisis line is staffed with trained volunteers and staff. The hotline is available 24 hours a day, 365 days a year, and receives over 12,000 calls per year. This is often the first contact a victim has with the agency and is an important life saving one. Immediate safety planning and crisis intervention counseling are provided. It is the first step in facilitating getting a victim out of a circumstance of abuse and into a safer situation. The hotline also offers resources and referrals to other agencies, law enforcement, legal, social service and medical support, and housing assistance. The 30-day crisis shelter is comprised of 6 units with a total of 36 beds, able to accommodate adults with or without their children. It is staffed 24 hours a day, 365 days a year. It is located at a confidential site and provides a classroom for the children staying at the shelter. A certified teacher is qualified to education K-12 children and the classroom offers all needed materials so that children are able to maintain their education and avoid missing valuable schooling. Individual and group counseling for victims and their children is an integral part of the program services. Other services offered are assistance with obtaining protective orders, providing advocacy for legal and social services, conducting life skill workshops, and providing food and clothing. The crisis shelter also offers basic medical assessments with a physician's assistant and health education. The outreach counseling program provides group counseling for survivors not needing crisis shelter services in both daytime and evening sessions with groups in English and Spanish. Part of the pattern of domestic violence is isolation and the feeling that you are the only person going through this. Some are still in the relationship and some are out, but all benefit from talking and learning from each other and the support provided. Subjects covered in the groups include self-esteem, the cycle of domestic violence, safety issues, and the sharing of resources with each other. Male and female victims can also access individual meetings with domestic violence advocates/counselors to also obtain legal and social service advocacy. The legal advocacy service component assists victims in meeting with an advocacy counselor and accessing the resources for restraining orders. The advocates are bilingual to meet the needs of Spanish speaking clients and will accompany the victim to court. The volunteer training program consists of a full-time training facilitator who conducts the 40-hour state mandated domestic violence training for individuals to receive their certificates to be able to work with victims of domestic abuse and their children. Haven Hills is certified to provide these trainings, not only to our employees and volunteers, but also offers the training for anyone who needs to obtain the certification.

IRS990/ProgSrvcAccomActy2Grp/Desc0The City of LA HCID Grant, CalOES, and CalWORKS Grants provide money to the Haven Two Transitional Program. The Haven Two Transitional Housing and Employment Program is an eighteen-month program provided to victims of domestic violence who have completed a stay in a domestic violence crisis shelter, are within low income guidelines, and are dedicated to achieving a level of self-sufficiency without abuse in their lives. The housing complex has 24 apartments ranging from 1 to 4 bedrooms, a children's play room, and counseling areas. At Haven Two, survivors learn job-training skills, continue their education and/or begin to work while continuing to address their domestic violence issues. Individual and group counseling for the adults and children is provided on site. The program is focused on helping the victim become self-sufficient, breaking the cycle of violence.
