The review and approval must occur at the time of the initial hiring and annually thereafter. The chairman prepares an annual performance evaluation of the President/CEO and shares it with the Executive Committee.”

“OTHER PROGRAM SERVICES 4: Family Caregiver Support Program - Offers a wide array of services to assist and support caregivers including case managers by providing training support workshops and respite services. Provides over approximately 2,000 hours of case management, runs 24 support groups and serves 325 caregivers annually. They service 4,594 clients and receive 3,657 calls annually. This program provided for 304 office visits and 21 home visits for support services. OTHER PROGRAM SERVICES 5: Enrichment Center(Adult Day Program) - Provides a safe and loving environment so client can enjoy socializing activities and lunch while their caregiver takes a full or half-day break. Provides 10,244 respite hours to 233 participants. OTHER PROGRAM SERVICES 6: Help Line - Provides information and referral services to seniors and their family. Receives an average of 1,200 calls for information and referrals from seniors and those who care for them annually. OTHER PROGRAM SERVICES 7: ReAL Connections Village - Neighborhood oriented, membership based program where people can access trusted resources and vetted services with one phone call. A mix of services would be offered that a majority of members would find available. Consumer driven and consumer run program helping people age in place in their neighborhoods. 125 members received 3,105 hours of service. The program has over 68 volunteers and provided approxiamtely 2,000 service requests. OTHER PROGRAM SERVICES 8: Connections Newsletter - a monthly publication that features information, current events, activities, topics of interest and volunteer opportunities. Available free at senior centers and various other locations. Approximately 3,300 copies are distributed to over 50 locations. OTHER PROGRAM SERVICES 9: Community Connections is an innovative volunteer driver program that provides door-through-door assisted transportation service to seniors and individuals with disabilities* who cannot easily use other transportation services. Community Connections encourages volunteer driver transportation for people who would otherwise be homebound and isolated. The key to the program is providing mileage reimbursement for volunteers to transport individuals when the individual is too frail, ill or unable to use public transportation for other reasons. Provides an average of 323 riders transportation to their life sustaining and life enriching destinations; over 362 volunteer drivers donate approximately 51,200 hours and travel 489,920 miles annually. OTHER PROGRAM SERVICES 10: Sr. Case Mgmt - Trained Case Manager assisted with questions and issues related to Medicare, HMOs, and Medicare Supplements, and long term care.”
