A conflict of interest disclosure statement is completed annually by each director, officer, and member of the executive staff, and at the time any such person assumes his or her position. The minutes of the meeting of the board or committee shall reflect that any potential conflict of interest was disclosed and that any interested party was not present during the final discussion or vote and did not vote. The board of directors and finance committee review this policy annually.”
