Ashley west (board president) provides legal representation to bryan merica's (director) company unearth campaigns. She is also legal counsel for garry maisel (director).

Form 990, Part VI, Section B, Line 11B

Each member of the board of directors receives a copy of the 990 prior to filing. The ceo, cfo, and cso review the 990 in detail and send the draft 990 to the finance committe, led by the treasurer of the board, to review prior to finalizing.

Board members and key staff are required to disclose any interests and sign a conflict of interest policy document annually. The executive committee of the board collects them, reviews them, and enforces compliance.

The ceo was hired in 2006 and her compensation was determined by the search team at that time. In 2009 a compensation committee of the board was created to review the ceo's salary. An external review was completed by the committee in 2009. No change was made to the ceo's compensation. In november 2015 the ceo's salary was reviewed and increased. All salaries are reviewed on an annual basis as a participant in a statewide review based on size types of services area etc.

PART X, LINE 2:

The organization has applied for and received a determination letter from the internal revenue service (irs) to be treated as a tax-exempt entity pursuant to section 501(c)(3) of the internal revenue code and did not have any unrelated business income for the years ended september 30, 2022 and 2021. Due to its tax-exempt status, the organization is not subject to income taxes. The organization is required to file tax returns with the irs and other taxing authorities. Accordingly, these financial statements do not reflect a provision for income taxes and the organization has no other tax positions which must be considered for disclosure. Organization is subject to routine audits by taxing jurisdictions; however, there are currently no audits for any tax periods in progress. Management believes it is no longer subject to income tax examinations for years prior to 2019.

CREEKSIDE VILLAGE IS THE ORGANIZATION'S PERMANENT SUPPORTIVE HOUSING PROGRAM FOR FAMILIES THAT ARE SURVIVORS OF DOMESTIC VIOLENCE.
