“Decisions are made by the Corporate Board, comprised of leadership members of the Daughters of Charity.”
“A complete copy of this Form 990 is provided to the Governing Board of Directors before filing the form. The Board reviews and discusses the Form 990 at their Board meeting and subsequently votes to accept the form.”
“Board Members must sign the Conflict of Interest Policy Statement acknowledging that they have received and read a copy of the policy and that they agree to abide by its terms. Each Board Member will execute this disclosure statement yearly to monitor potential future conflicts. The term "conflict of interest" describes a situation in which a Board Member's own interest may influence the way that Board Member handles Agency business. The person must, therefore, refrain from all actions that impair, or provide a reasonable perception of impairing independence or judgement and report all conflicts or potential conflict of interest.”
“The Board of Directors reviews and recommends compensation for the Executive Director. The Board of Directors approves the annual budget which includes compensation information for all employees.”
“Mount St. Joseph-St. Elizabeth makes available to the public upon request information about its governing documents, conflict of interest policy, and financial statements.”
“Epiphany Center programs strengthens family life through early interventions when possible and by healing trauma, treating mental illness and substance abuse, and ending cycles of homelessness and poverty through holistic client-centered care. Epiphany Center provides holistic client-centered care to a diverse population of children, women, and families who are the most vulnerable in our society. Our purpose is to strengthen family life and to enhance the physical, social-emotional, intellectual and spiritual growth of each person in our care. Epiphany Center provides a variety of services to family from preventative services, including In Home Services which uses the SafeCare model of assessing and providing services to families in their own homes. With funding from the Office of Child Abuse Prevention, Epiphany Center's Road to Resilience project helps women who are pregnant and/or have infants 0-12 months of age to find safety and stability through our services and referrals. The Road To Resilience team recognizes that each mother and child has unique needs. We support high-risk pregnant women with current and/or a history of substance abuse, as well as families of substance-exposed infants, to get the help they need to achieve their goals. Our Family Treatment Program provides infant mental health services to babies who may have been prenatally exposed to drugs or suffer from other traumas, requiring special intervention therapies. We also provide a Family Enrichment Program to enhance the relationships between mothers and their children, especially in effectively utilizing unstructured time together.”
“On July 1, 2019, Epiphany Center began our Road to Resilience Program, the result of being rewarded one of only 10 government grants given to agencies to go into the community and provide services to pregnant women or mothers of infants under 12 months of age with a history of substance abuse. Our Navigators worked with 55 women and their infants, coordinating services with several collaborating agencies who provide services to women, such as emergency housing, medical services, and residential treatment centers. Our Parent Counselor works with the Navigator and provides Prenatal and Postnatal care to the mothers and helps them develop the mother-baby bond. Many of the women and babies are able to be referred to Epiphany Center's residential treatment and the babies to our daycare program where we have dedicated two teachers to work with the women who participate in the Road to Resilience program. The majority of Road to Resilience clients are able to receive the supports they need to stay in the community. The Road to Resilience grant is a 3 year grant with a possible 2 year extension, dependent on outcomes and funding availability.”
“A portion of the pooled securities of Fund P are permanently restricted endowed assets which will be held in perpetuity. Investment earnings related to these permanently restricted assets are available for the operations of the organization.”
“Epiphany Center is exempt from taxation under Internal Revenue Code Section 501(c)(3) and California Revenue and Taxation Code Section 23701d. Generally accepted accounting principles provide accounting and disclosure guidance about positions taken by an organization in its tax returns that might be uncertain. Management has considered its tax positions and believes that all of the positions taken by the Epiphany Center in their federal and state exempt organization tax returns are more likely than not to be sustained upon examination. Epiphany Center's returns are subject to examination by federal and state taxing authorities, generally for three and four years, respectively, after they are filed.”

| IRS990/Desc | 0 | Epiphany Center is a 3.1, 3.3, 3.5 ASAM (American Society of Addiction Medicine) designated facility offering holistic residential services to women struggling with addiction. We ascribe to the medical model of addiction as well as follow a harm reduction philosophy. We are Trauma Informed in our comprehensive care for our highly traumatized population. Our staff receive ongoing training and thus have keen understanding and insight into the neurological, biological, psychological, and social effects of trauma and the pervasiveness of these experiences in persons who seek and receive addiction recovery services.A unique feature that we provide is the capacity for our clients to enter our Program with their children (or in the midst of child reunification) ages 0-3. While the women are actively engaged in their recovery program, the children's unique needs are simultaneously being served through our Children Services Program. Further, the mother-child dyad is also served through our Family Treatment department offering a range of individual and family therapy including a focus on infant mental health. We have developed our very unique Wellness platform whereby we address every aspect of a client's individual wellness desires, concurrent with her recovery and mental health goals. Our holistic approach is unique, and we believe, leads to a more comprehensive and strength based recovery.During the year the program served 132 women and 20 children. |

| IRS990/ProgSrvcAccomActyOtherGrp/Desc | 3 | On July 1, 2019, Epiphany Center began our Road to Resilience Program, the result of being rewarded one of only 10 government grants given to agencies to go into the community and provide services to pregnant women or mothers of infants under 12 months of age with a history of substance abuse. The Road to Resilience grant is a 3 year grant with a possible 2 year extension, dependent on outcomes and funding availability. |
