The Form 990 is prepared internally under the review of the President and Treasurer. The completed Form 990 is distributed to the Board for review prior to filing with the IRS.

Form 990, Part VI, Section B, Line 12C

Each year the employees and Board members will receive a Conflict of Interest form to complete, as well as a copy of the Conflict of Interest Policy. The completed forms will be kept in a confidential file in the President's office. Regular follow ups with employees and Board members will occur during the year to ensure all Conflict of Interest forms are comlpeted.
