Board policy dictates that transactions involving board members with conflicts of interest are to be fully disclosed and subject to board approval. Conflicts of interest inquiries are made at employee orientation and regularly each year. Both management and employees are required to disclose any possible conflicts. Employee submissions are reviewed by hr. Necessary conflicts are submitted to the executive leadership team for review who will bring it to the board of directors on a needed basis.

All board members are volunteers. We also have patient advisory councils (pac) that meet once a month per clinic. Our bend clinic also has a spanish pac that meets once a month.

Form 990, Page 2, Part III, Line 4D

The redmond clinic provided 10,455 visits, as well as additional connections to dental, medical assistance, and case management services in fiscal year 2016-2017. Additionally, our redmond clinic maintained a state of oregon level four designation for patient-centered primary care homes. The madras clinic provided 9,118 visits, as well as additional connections to dental, medical assistance, hospital ob and call services, and case management services in fiscal year 2016-2017. In addition, our madras clinic maintained a state of oregon level four designation for patient- centered primary care homes. Our school-based health centers provided 5,513 visits, as well as additional connections to dental, medical assistance, and case management services in fiscal year 2016-2017. Mosaic medical provided the balance of our visits through our two satelite sites, harriman health and ariels community clinic, and our two mobile vans, mosaic medical van and mosaic dental van.

Schedule D, Page 3, Part X

The organization is exempt from taxation under section 501(c)(3) of the internal revenue code (irc) and has been classified as an organization that is not a private foundation under section 509(a)(2). No provision for income taxes is made in the accompanying financial statements, as the organization has no activities subject to unrelated business income tax. The organization files income tax returns in the united states federal jurisdiction, as well as the state of oregon jurisdiction. In the event penalties and interest are assessed by income taxing authorities, it is the organizations policy to include these in operating expenses. No penalties or interest were assessed for the years ended may 31, 2017.
