Mission and Program Overview

Mission

CCIH provides permanent affordable housing coupled with well-executed support services to people who are homeless or at risk to become homeless in Contra Costa County. Support services are individualized for each person or family in order to assist them to stabilize their health and housing and become as self-sufficient as possible. Adult services focus on mental and physical health, life skills, employment, parenting, and eviction prevention. Youth services focus on emotional and physical health, education, social and life skill building, good citizenship and preparation for adulthood. All services are accessible in that they are provided on-site at apartment complexes or in consumers' homes.

The mission of CCIH is to provide permanent, affordable housing and vital support services to homeless and at-risk families and individuals in Contra Costa County.

Governance Explanations

Form 990, Part VI, Section A, line 6

Qualifications - Each church/temple/mosque/faith-based/charitable, service, non-profit corporation who pays dues shall be a voting member of the Corporation. Classes of Membership - There shall be only one class of members of the Corporation. Admission to Membership - Board of Directors shall from time-to-time prescribe the form, manner and timing for application for membership. Dues - Annual dues, which shall be in the amount established by the Board of Directors on or before May 1st of each year, shall be payable in advance on or before the annual meeting each year. Transfer of Memberships - Under no circumstances may a membership in the Corporation be transferred.

Form 990, Part VI, Section A, line 7A

The Corporation shall have up to twenty-five (25) Directors, who shall be known as Directors; collectively, the Directors shall be known as the Board of Directors. The slate of candidates shall include the person nominated by each faith congregation. One third of the Board of Directors shall be elected each year. The Directors shall be elected by the members at the Annual Meeting. Once the candidates for Directors have been nominated, the members present at the meeting shall elect so many Directorships as are then up for election. A plurality of the votes cast for each such Directorship shall elect the Director.

Form 990, Part VI, Section B, line 12C

CCIH Board members and staff disclose conflicts of interest by completing the "Conflict Disclosures Questionnaire Form" on an annual basis at the beginning of the fiscal year and promptly when a new conflict is identified. This disclosure form requests individuals to identify their interests that could give rise to conflicts of interest, such as a list of family members, substantial business or investment holdings, and other transactions or affiliations with businesses and other organizations or those of family members as well as other nonprofit organizations. Board and staff are also urged to disclose conflicts as they arise and to disclose those situations that are evolving that may result in a conflict of interest. Advance disclosure must occur so that a determination may be made as to the appropriate plan of action to manage the conflict. Staff should disclose to the Executive Director and board members should disclose to the Board or Chairperson of the board as soon as they are aware of an actual or potential conflict of interest, holdings, and other transactions or affiliations with businesses and other organizations or those of family members as well as other nonprofit organizations.

CCIH does a compensation survey of local non-profit agencies to ensure that the ED salary is not out of line with local standards. The survey process is overseen by an adhoc committee consisting of Board members.

Part X, Line 2:

CCIH is exempt from taxation under Internal Revenue Code Section 501(c)(3) and California Revenue and Taxation Code Section 23701(d). Generally accepted accounting principles provide accounting and disclosure guidance about positions taken by an organization in its tax returns that might be uncertain. Management has considered its tax positions and believes that all of the positions taken by CCIH in their federal and state exempt organization tax returns are more likely than not to be sustained upon examination. CCIH's returns are subject to examination by federal and state taxing authorities, generally for three and four years, respectively, after they are filed.

IRS990/MissionDesc0CCIH provides permanent affordable housing coupled with well-executed support services to people who are homeless or at risk to become homeless in Contra Costa County. Support services are individualized for each person or family in order to assist them to stabilize their health and housing and become as self-sufficient as possible. Adult services focus on mental and physical health, life skills, employment, parenting, and eviction prevention. Youth services focus on emotional and physical health, education, social and life skill building, good citizenship and preparation for adulthood. All services are accessible in that they are provided on-site at apartment complexes or in consumers' homes.

CCIH houses 48 formerly chronically homeless individual adults (many of whom are seniors) with special needs. Additionally, CCIH also housed 21 formerly chronically homeless families in the same scattered site model. A mobile service team of case managers provide services to ensure their stability, physical and mental well-being, and to assist them in retaining their housing and increasing life skills. The children in this program receive intensive academic support, and have access to mental health counseling.
