Mission and Program Overview

Mission

Transforming the lives of individuals and their communities through innovative and effective addiction and social services

In continuing the mission of drug treatment and research and responding to the increases in statewide heroin addiction and overdose death rates, ets has been asked by local governmental entities to significantly expand operations to meet these growing needs. Ongoing expansion efforts started in 2014 and are expected to be completed late in the next fiscal year. The resulting large capital outlays and significantly increased operating expenses in the last two years are expected to be offset by increased treatment revenues in future as they strive to meet the current and future demands for their treatment services.

Governance Explanations

Form 990, Part VI, Section B, Line 11

The form 990 will be reviewed by the finance director. Once approved by them, the form 990 is routed to the board of directors for comment. Any feedback received during the review process is incorporated into the final version that gets filed with the internal revenue service.

Form 990, Part VI, Section B, Line 12C

Evergreen treatment services (ets) requires that all officers, directors and chief executive and chief financial executive complete an annual conflict of interest statement. Statements are reviewed by the chairman of the board of directors who determines whether there are any potential conflicts of interest and takes the required action based on the conflict. Additionally, ets' chief executive and chief finance executive have the responsibility of monitoring proposed or ongoing transactions for conflicts of interest for possible disclosure to the board of directors in order to deal with potential or actual conflicts. In the event a conflict is determined to exist, the most common monitoring and enforcement method involves excluding the individual from participating in the discussion and all decisions and votes affecting the area of conflict.

Form 990, Part VI, Section B, Line 15A

The process for determining compensation of ets' chief executive includes comparable compensation information such as the annual united way survey as well as review of compensation for similarily qualified people in funcationally comparable positions at similar organizations. Final compensation is subject to review and approval of the board of directors on an annual basis.

Form 990, Part VI, Section C, Line 19

Evergreen treatment services ("ets") makes forms 1023 and 990 available to the public upon request. Ets also provides copies of its governing documents, conflict of interest policy and financial statements to the public as required.

IRS990ScheduleO/SupplementalInformationDetail/ExplanationTxt0THE FORM 990 WILL BE REVIEWED BY THE FINANCE DIRECTOR.
IRS990ScheduleO/SupplementalInformationDetail/ExplanationTxt1EVERGREEN TREATMENT SERVICES (ETS) REQUIRES THAT ALL OFFICERS, DIRECTORS AND CHIEF EXECUTIVE AND CHIEF FINANCIAL EXECUTIVE COMPLETE AN ANNUAL CONFLICT OF INTEREST STATEMENT. IRS990ScheduleO/SupplementalInformationDetail/ExplanationTxt5EVERGREEN TREATMENT SERVICES USES VOLUNTEERS TO SERVE ON ITS BOARD OF DIRECTORS. PERIODICALLY, VOLUNTEERS ARE ALSO USED TO PERFORM A VARIETY OF NEEDED OTHER BUSINESS ACTIVITIES AS SUCH VOLUNTEERS BECOME AVAILABLE TO ETS.
