The membership of this corporation shall consist of only the regular salaried members of the phoenix, glendale, tempe, chandler and peoria fire department who are in good standing with the united phoenix fire fighters union, local 493.

Has received a copy of the conflicts of interest policy, b. Has read and understands the policy, c. Has agreed to comply with the policy, and d. Understands the organization is charitable and in order to maintain its federal tax exemption it must engage primarily in activities which accomplish one or more of its tax-exempt purposes procedures for addressing the conflict of interest: an interested person may make a presentation at the governing board of committee meeting, but after the presentation, he/she shall leave the meeting during the discussion of, and the vote on, the transaction or arrangement involving the possible conflict of interest.

At this time the organization does not compensate officers or key employees for adminstrative work, however if in the future this changes, the organization's process for determining compensation will includes a review and approval by independent persons, comparability data, and recording of the discussion and the decision.

The organization will make their application for recognition of exemption and annual information returns available upon request without charge except for a nominal fee for reproduction and actual postage costs. Annual information returns will be available for three years after filing.

Part VIII, Line 1F

On june 30, 2013 the yarnell hill fire in yarnell, arizona overran and killed 19 members of the granite mountain hotshots. Through the outpouring of community support more than $3,300,000 was raised. These funds were transferred to prescott fire fighters charity for disbursement to the families as appropriate.
