Casa de la Luz Statement of Vision, Mission and Activities The vision for Casa de la luz Foundation is to empower the community, patients and their loved ones, to transform the end of life experience into the final act of living well. The organization does this by providing supplemental financial support to hospice patients and their families. This support can range from a few dollars for toiletries or transportation for a spouse to visit their loved one in a care facility, to paying for a months utilities so an indigent patient can realize their dream of dying at home or assisting a family with cremation. The Foundation is also a trusted and central educational resource on end of life care issues for Tucson and Southern Arizona. Through its annual End of Life community Conference to workshops on understanding the value of hospice and palliative care, Casa de la Luz Foundation provides education and training to care givers, physicians, faith leaders, attorneys, and community members. A critical part of the Foundations education program is being the primary local distributer of Five Wishes, the most widely used advance directive or living will in America. Casa Foundation participates in a community collaboration, the End of Life Care Partnership, that provides workshops on the importance of having advanced directives.

IRS990ScheduleO/SupplementalInformationDetail/ExplanationTxt4Casa de la Luz Statement of Vision, Mission and Activities The vision for Casa de la luz Foundation is to empower the community, patients and their loved ones, to transform the end of life experience into the final act of living well. The organization does this by providing supplemental financial support to hospice patients and their families. This support can range from a few dollars for toiletries or transportation for a spouse to visit their loved one in a care facility, to paying for a months utilities so an indigent patient can realize their dream of dying at home or assisting a family with cremation. The Foundation is also a trusted and central educational resource on end of life care issues for Tucson and Southern Arizona. Through its annual End of Life community Conference to workshops on understanding the value of hospice and palliative care, Casa de la Luz Foundation provides education and training to care givers, physicians, faith leaders, attorneys, and community members. A critical part of the Foundations education program is being the primary local distributer of Five Wishes, the most widely used advance directive or living will in America. Casa Foundation participates in a community collaboration, the End of Life Care Partnership, that provides workshops on the importance of having advanced directives.
